摘  要:目的观察听性脑干反应(auditory brainstem response, ABR)未引出的婴幼儿耳蜗微音电位(cochlear microphonics potential,CM)和畸变产物耳声发射(distortion product otoacoustic emission,DPOAE)的引出率,研究CM在婴幼儿听神经病(auditory neuropathy,AN)诊断中的应用价值。方法选取在100 dB nHL click-ABR未引出的婴幼儿55例(110耳)为研究对象,年龄为3个月~6岁9个月,平均22.40±18.63个月,记录CM和DPOAE的引出率,以ABR未引出、DPOAE和/或CM引出为AN的诊断标准。结果 55例(110耳)中,DPOAE和CM的引出率分别为2.72%(3/110)和16.36%(18/110),其中,2例(3耳)DPOAE和CM均引出,9例(15耳)CM引出但DPOAE未引出;10例(18耳)诊断为AN。结论当ABR和DPOAE均未引出时,要常规行CM检查,避免AN的漏诊。Objective To study the elicited ratio of cochlear microphonics potential(CM)and distortion product otoacoustic emission(DPOAE)in infant whose auditory brainstem response was not evoked.Methods Fifty five infants from three months old to eighty one months old showed no auditory brainstem response between January 2019 and October 2019 were tested with CM and DPOAE.Results DPOAE was recorded in 3 ears(2.72%),CM was recorded in 18 ears(16.36%),DPOAE and CM were both recorded in 2 cases(3 ears),CM was recorded but DPOAE was not recored in 9 cases(15 ears).Auditory neuropathy was diagnosed in 10 cases(18 ears).Conclusion When no response in ABR and DPOAE,we recommend CM test routinely to avoid missed diagnosis of the auditory neuropathy.
