同期经尿道膀胱肿瘤电切术与经尿道前列腺切除术治疗浅表性膀胱癌合并良性前列腺增生的效果

摘　　要：目的探讨同期经尿道膀胱肿瘤电切术(TURBT)与经尿道前列腺切除术(TURP)治疗浅表性膀胱癌合并良性前列腺增生的效果。方法选取我院2019年1月~2020年1月收治的36例浅表性膀胱癌合并良性前列腺增生患者作为研究对象,按照随机数字表法分为对照组(18例)与观察组(18例)。对照组应用TURBT治疗,观察组采用同期TURBT与TURP治疗。比较两组的手术指标、残余尿量变化情况、临床疗效与并发症总发生率。结果观察组的手术时间长于对照组,术中出血量多于对照组,且术后留置导尿管时间短于对照组(P<0.05);两组术后的残余尿量少于术前,且观察组术后的残余尿量少于对照组(P<0.05)。观察组的总有效率为94.44%,高于对照组的66.67%(P<0.05)。观察组的并发症总发生率为5.56%,低于对照组的33.33%(P<0.05)。结论浅表性膀胱癌合并良性前列腺增生患者接受同期TURBT与TURP治疗后,可显著减少残余尿量,并降低并发症发生率。Objective To investigate the effect of simultaneous transurethral resection of bladder tumor(TURBT)and transurethral resection of prostate(TURP)in the treatment of superficial bladder cancer with benign prostatic hyperplasia.Methods A total of 36 patients with superficial bladder cancer combined with benign prostatic hyperplasia admitted to our hospital from January 2019 to January 2020 were selected as study subjects,and they were divided into the control group(18 cases)and the observation group(18 cases)according to the random number table method.TURBT treatment was applied to the control group,while TURBT and TURP treatment were applied to the observation group.Surgical indexes,changes in residual urine volume,clinical efficacy and total incidence of complications were compared between the two groups.Results The operative time of the observation group was longer than that of the control group,the intraoperative blood loss of the observation group was more than that of the control group,and the duration of indwelling catheter of the observation group was shorter than that of the control group(P<0.05).After surgery,the residual urine volume of the two groups was less than that before operation,and the residual urine volume in the observation group after treatment was less than that in the control group(P<0.05).The total effective rate of the observation group was 94.44%,which was higher than 66.67%in the control group(P<0.05).The total incidence rate of complication of the observation group was 5.56%,which was lower than 33.33%of the control group(P<0.05).Conclusion After the treatment of TURBT and TURP at the same time,the amount of residual urine and the incidence of complications can be significantly reduced in the patients of superfical bladder cancer complicated with benigh prostatic hyperplasia.
