摘　　要：目的探讨DSA引导下经鼻型肠梗阻导管治疗术后小肠粘连性梗阻的可行性和临床疗效。方法回顾性分析2016年6月~2019年10月在我院接受经鼻型肠梗阻导管治疗粘连性肠梗阻75例患者,观察患者梗阻症状及体征变化、自主排气、排便时间、24h引流量以及影像学检查评价等指标,并与传统的鼻胃管减压方法相比较,并分析手术并发症发生情况。比较两组患者的治疗效果并评价经鼻型肠梗阻导管在治疗术后粘连性肠梗阻的临床价值。结果所有患者均成功置管,技术成功率100%。与传统鼻胃管比较,经鼻型肠梗阻导管能够快速改善梗阻症状,缩短病程,明显提高患者的治愈率,差异有统计学意义(P<0.05)。结论DSA引导下置入经鼻型肠梗阻导管治疗粘连性肠梗阻安全快捷,较传统鼻胃管减压梗阻症状改善更加快速高效,提高保守治疗的成功率。Objective The clinical application value and feasibility of DSA-guilded transnasal ileus tube placement in the treatment of adhesive small intestinal obstruction.Methods From June 2016 to October 2019,75 patients with simple adhesive small intestinal obstruction were discussed in the DSA-guilded transnasal ileus tube and compared with the traditional methods of decompression of nasogastric tube.Symptoms and signs of obstruction,self exhaust and defecation time,drainage flow within 24 h,complication were compared between groups.The treatment effect in the two groups was compared to evaluate clinical application value of ileus tube.Results All patients were inserted successfully that improving the symptoms of obstruction,shortened the course of disease,and significantly improved the cure rate of patients compared to gastric tube.The difference was statistically significant(P<0.05).Conclusion DSA-guilded transnasal ileus tube placement in the treatment of adhesive small intestinal obstruction is safe and efficient,which is faster and more efficient than traditional nasogastric tube decompression obstruction symptoms,and improves the success rate of conservative treatment.
