早孕期胎儿面部特征性超声检查指标对18-三体胎儿诊断价值的研究

摘　　要：目的评估早孕期胎儿面部特征性超声检查指标对18-三体胎儿诊断的价值。方法回顾性分析41例产前确诊为18-三体胎儿的孕早期超声检查图像,测量相关指标,总结各指标与该疾病的相关性。结果41例18-三体胎儿的平均鼻骨长度(NBL)、上颌-鼻根-下颌角(MNM)分别是1.7 mm,19.30°,51.2%胎儿面部轮廓线(FP)是0,46.1%是负,仅1例为正。所有标记均与18-三体有明确相关性。18-三体胎儿的检出率NBL为63.7%,MNM角为53.4%,FP线为46.0%。上述三种标记(NBL,FP,MNM)组合导致检出率在92.7%。结论早孕期胎儿面部特异性超声检查指标联合对于18-三体胎儿的早期诊断有肯定作用,可提高诊断正确率。Objective To evaluate the diagnostic value of facial relevant ultrasound markers of trisomy 18 fetus in the first trimester.Methods The ultrasound images of 41 fetuses diagnosed as trisomy 18 in the first trimester were analyzed retrospectively,and the relevant markers were measured,and the correlation between each marker were summarized.Results In the confirmed 41 cases of trisomy 18 fetuses,the average of NBL and MNM was 1.7 mm,19.30°degrees respectively.51.2%of FP line were 0,46.1%of cases were negative,only one case was positive.All markers had a clear correlation with trisomy 18.The detection rate of the trisomy 18 fetuses was as follows:NBL was 63.7%,MNM Angle was 53.4%,and FP line was 46.0%.The detection rate by combination of the above three markers(NBL,FP,MNM)was elevated to 92.7%.Conclusion The combination of these facial relevant ultrasound markers has positive effect on the early diagnosis of trisomy 18,which could improve the rate for the diagnosis of trisomy 18 in the first trimester of pregnancy.
