“益气和胃、通腑泻浊法”治疗急性胰腺炎理论探讨

摘　　要：急性胰腺炎(Acute Pancreatitis,AP)属于临床急腹症,且多合并不同程度的胃肠功能障碍,近年来,随着人们生活水平的提高,其发病率呈上升趋势,若病情恶化可进展为重症急性胰腺炎,其临床病死率可高达20%~40%,临床形势严峻。本病属于中医“脾心痛”范畴,中医药治疗急性胰腺炎优势明显。临床上研究组采用中西医结合方式治疗急性胰腺炎,中医方案采用益气和胃、通腑泻浊法上下同治,取得良好疗效。且研究组前期临床研究已经证明此方案能提高急性胰腺炎的临床疗效,改善急性胰腺炎患者的症状体征,降低EPIC评分、APACHEⅡ评分,缩短患者住院天数。本文就此中医方案进行阐述,以期为临床采用“益气和胃、通腑泻浊法”上下同治方案治疗急性胰腺炎奠定理论基础,并在以后的研究中进一步探索该方案对胰腺炎的具体药理机制。Acute pancreatitis belongs to clinical acute abdomen,and it has different gastrointestinal dysfunction.In recent years,with the improvement of living standards,its morbidity increases.If the condition deteriorates,it can progress to severe acute pancreatitis,and its clinical mortality can be as high as 20%to 40%,therefore the clinical situation is severe.The disease belongs to the category of“spleen-heart ache”in traditional Chinese medicine.Clinically,the research group used the combination of traditional Chinese and western medicine to treat acute pancreatitis.The traditional Chinese medicine program adopted the method of Replenishing Qi and stomach,relaxing the bowels and expulsion of toxin with nasal feeding and enema,and achieved good curative effect.And the preliminary clinical research of the research group has proved that this program can improve the clinical efficacy of acute pancreatitis,reduce the symptoms and signs,reduce the EPIC score,APACHE Ⅱ score,and shorten the length of hospital stay.This article elaborates on this method,in order to lay a theoretical foundation for the clinical treatment of acute pancreatitis by using“Replenishing Qi and stomach,relaxing the bowels and expulsion of toxin”method,and further explore the specific pharmacological mechanism of this program for pancreatitis in future research.
