软肝化症汤加减辨证治疗肝硬化腹水的临床疗分析及对肝纤化指标的影响

摘　　要：目的:观察软肝化症汤治疗肝硬化腹水的临床疗效及对肝纤化指标的影响。方法:以2017年4月~2019年4月来我院感染科就诊的肝硬化腹水患者为研究对象,选取96例,随机分为2组,每组各48例。对照组患者采用安慰剂结合常规基础治疗,观察组患者软肝化症汤结合常规基础治疗,两组均2w为1疗程,连续治疗2疗程。观察治疗前后两组患者中医证候积分、临床综合疗效、肝纤化指标、基本临床特征、肝硬化Child-Pugh分级。结果:治疗后两组患者治疗后脘腹痞胀、水肿、便溏、恶心呕吐等证候评分均显著降低(P<0.05);并且观察组治疗后的中医证候积分较对照组低(P<0.05)。观察组的临床总有效率显著高于对照组(P<0.05);两组患者治疗前后肝硬化Child-Pugh分级经秩和检验,Z=-3.384,P<0.05,差异有统计学意义。治疗1w及2w后观察组患者的腹围、体质量、肝前腹水及下腹部腹水显著降低(P<0.05),24h尿量明显增多(P<0.05),较对照组改善更为明显(P<0.05),治疗后观察组PCIII、HA、LN及CIV明显低于对照组,两组比较具有统计学意义(P<0.05)。结论:软肝化症汤治疗肝硬化腹水临床疗效显著,值得临床推广使用。Objective:To evaluate the clinical effect of using Ruangan Huazheng Decoction in the treatment of ascites due to cirrhosis and its influence on liver fibrosis index.Methods:From April 2017 to April 2019,96 patients with ascites due to cirrhosis were randomly divided into two groups,48 in each group.The patients in the controlled group were treated with placebo combined with conventional basic treatment,and the patients in the observation group were treated with Ruangan Huazheng Decoction combined with conventional basic treatment.The two groups were treated for one course of treatment for two consecutive courses of treatment for two weeks.Before and after treatment,TCM syndrome score,clinical comprehensive effect,liver fibrosis index,basic clinical characteristics,Child Pugh classification of cirrhosis were observed.Results:After treatment,the scores of abdominal distention,edema,loose stools,nausea and vomiting in the two groups were significantly reduced(P<0.05),and the scores of TCM syndromes in the observation group were lower than those in the controlled group(P<0.05).The total clinical effective rate of the observation group was significantly higher than that of the controllede group(P<0.05).After 1 and 2 weeks of treatment,the peritoneum,body mass,pre liver ascites and lower abdominal ascites of the patients in the observation group were significantly reduced(P<0.05),the urine volume in 24 hours was significantly increased(P<0.05),and the improvement was more obvious than that in the controlled group(P<0.05).After treatment,PCIII,ha,LN and civ in the observation group were significantly lower than those in the controlled group(P<0.05).Conclusion:The clinical effect of using Ruangan Huazheng Decoction in the treatment of cirrhosis ascites is significant,and it is worthy of clinical application.
