健脾疏肝汤联合文拉法辛治疗复发性抑郁障碍疗效及对脑源性神经营养因子、抗氧化能力的影响

摘　　要：目的:观察健脾疏肝汤联合文拉法辛治疗复发性抑郁障碍疗效及对脑源性神经营养因子、抗氧化能力的影响。方法:选择我院2019年1月~2020年3月收治的复发性抑郁障碍患者90例,按照随机数字表法分为2组,每组45例,对照组予以文拉法辛缓释胶囊口服,治疗组在对照组用药基础上予以中药健脾疏肝汤口服,疗程均为8w。比较两组治疗前后汉密尔顿抑郁量表(HAMD)-17评分、罗森伯格自尊量表(SES)评分、总体幸福感量表(GWB)评分,并比较两组血清脑源神经营养因子(BDNF)、尿酸(UA)水平,并比较两组临床疗效。结果:治疗后治疗组HAMD-17各维度评分和总评分均低于对照组(P<0.01),SES、GWB评分均高于对照组(P<0.01),血清BDNF、UA水平均高于对照组(P<0.01),临床疗效优于对照组(P<0.05),差异均有统计学意义。结论:健脾疏肝汤联合文拉法辛有利于有效缓解抑郁症状,提高患者自尊程度和幸福度,治疗复发性抑郁障碍效果显著,其机制可能与于改善机体抗氧化能力,提高BDNF的血液含量有关。Objective:To investigate the effect of Jianpi Shugan Decoction combined with Venlafaxine on recurrent depression and its influence on brain-derived neurotrophic factor and antioxidant capacity.Methods:90 patients with recurrent depressive disorder admitted to our hospital from January,2019 to March,2020 were divided into 2 groups according to the digital random table method,with 45 patients in each group.The controlled group were given Venlafaxine Sustained-release Capsule orally,the treatment group were treated with Jianpi Shugan Decoction based on the treatment of the controlled group,and the treatment course was 8 weeks.The Hamilton Depression Scale(HAMD)-17 score,Rosenberg Self-esteem Scale(SES)score and general well-being scale(GWB)score before and after treatment were compared between the two groups,and the levels of brain-derived neurotrophic factor(BDNF)and uric acid(UA)of the two groups were compared,and the clinical effect was compared.Results:HAMD-17 scores and total scores of the treatment group were lower than those of the controlled group after treatment(P<0.01),SES and GWB scores were higher than those of the controlled group(P<0.01),serum BDNF and UA levels were higher than those of the controlled group(P<0.01),the clinical efficacy was better than that of the controlled group(P<0.05),and the differences were statistically significant.Conclusion:Jianpi Shugan Decoction combined with Venlafaxine can effectively alleviate depression symptoms,improve self-esteem and well-being of patients,and it has a significant effect on the treatment of recurrent depression.The mechanism may be related to the improvement of antioxidant capacity and blood content of BDNF.
