芎芷煎方对偏头痛患者血清NO、5-HT及血液流变学的影响

摘　　要：目的:观察芎芷煎方对偏头痛患者血清一氧化氮(NO)、5羟色胺(5-HT)以及血液流变学影响。方法:选取2018年10月~2019年10月诊治的88例偏头痛患者随机分为对照组与观察组,每组44例。对照组给予盐酸洛美利嗪5mg口服,日2次,持续治疗4w;观察组加服中药芎芷煎方汤剂,持续治疗4w。检测实验室指标[内皮素-1(ET-1)、一氧化氮(NO)、5-羟色胺(5-HT)]、血液流变学变化情况,并从疼痛程度、持续时间、发作频率来评价头痛症状评分,比较临床疗效。结果:与治疗前比较,两组血清ET-1水平降低(P<0.01),NO、5-HT水平升高(P<0.01),高切全血黏度、低切全血黏度、血浆黏度、红细胞压积水平降低(P<0.01),头痛疼痛程度、持续时间、发作频率评分降低(P<0.01);与对照组比较,观察组血清ET-1水平较低(P<0.01),NO、5-HT水平较高(P<0.01),高切全血黏度、低切全血黏度、血浆黏度、红细胞压积水平较低(P<0.01),头痛疼痛程度、持续时间、发作频率评分较低(P<0.01),总有效率较高(P<0.01)。结论:盐酸洛美利嗪联合芎芷煎方治疗偏头痛疗效满意,可能与上调NO、5-HT表达以及改善血液流变学有关。Objective:To investigate the effect of Xiongzhijian Recipe on serum nitric oxide(NO),5-hydroxytryptamine(5-HT)and hemorheology in migraine patients.Methods:88 patients with migraine from October 2018 to October 2019 were randomly divided into controlled group and experimental group,44 cases each.The controlled group received Lomerizine Hydrochloride 5mg,bid,and were treated for 4 weeks,the experimental group received more with Xiongzhijian Recipe for 4 weeks.The levels of endothelin-1(ET-1),nitric oxide(NO),5-hydroxytryptamine(5-HT)and hemorheology were compared,and the headache symptom score was evaluated from pain degree,duration and attack frequency to compare the clinical effect.Results:Compared with before treatment,the levels of serum ET-1,NO and 5-HT in the two groups decreased(P<0.01),high shear whole blood viscosity,low shear whole blood viscosity,plasma viscosity,hematocrit level were lower(P<0.01),headache pain degree,duration,attack frequency score were lower(P<0.01),the total effective rate was higher(P<0.01).Conclusion:Xiongzhi Decoction has a satisfactory therapeutic effect on migraine,which may be related to the up regulation of NO,5-HT expression and the improvement of hemorheology.
