光活化消毒在牙周牙髓联合病变治疗中的应用研究

摘　　要：目的评价应用光活化消毒(PAD)辅助牙周牙髓联合病变治疗的临床疗效。方法选取30例牙周牙髓联合病变患者随机分成三组,常规根管治疗+常规牙周治疗(A组),PAD辅助根管治疗+常规牙周治疗(B组),PAD辅助根管治疗+PAD辅助牙周治疗(C组)。观察综合治疗前(T1)、综合治疗后6周(T2)和综合治疗后12周(T3)时牙周探诊深度(PD)、临床附着水平(CAL)、探诊出血(BOP)的变化;随机选择患牙牙周内两个位点(PD≥5mm)采集样本,冷冻保存后使用Real-time PCR技术检测三组指标数在T1、T2、T3时牙龈卟啉单胞菌(porphyromonas gingivalis,Pg)相对总菌含量的变化。结果综合治疗后6周和12周时,A、B、C三组的PD、CAL、BOP均较治疗前显著下降;C组PD、CAL、BOP指标在治疗后6周和12周数值均低于同期A组和B组,差异具有统计学意义(P<0.05);Pg相对总菌的含量在治疗第6周和第12周时,C组Pg相对总菌的含量低于A组和B组,三组组间Pg相对总菌的含量差异有统计学意义(P<0.05);牙周牙髓联合病变的治疗效果分析,综合治疗12周后,A组与B组、B组与C组比较差异无统计学意义,A组与C组比较时,差异有统计学意义(P<0.05)。结论光活化消毒辅助牙周牙髓联合病变的治疗疗效显著。Objective To evaluate the adjunctive effect of Photo-Activated(PAD)for the treatment of periodontal-endodontic combined lesions.Methods 30 cases of periodontal-endodontic combined lesions patients randomly divided into three groups,group A:conventional root canal treatment and conventional periodontal treatment;Group B:PAD auxiliary conventional root canal treatment and conventional periodontal treatment;Group C:PAD auxiliary root canal treatment and periodontal treatment.Observe the change of periodontal probing depth(PD),clinical attachment level(CAL),bleed on probing(BOP)before comprehensive treatment(T1)and after treatment for 6 weeks(T2),12 weeks(T3).Randomly select two loci(PD≥5 mm)from patient periodontal as sample,Using real-time PCR technology to detect the relative content changes of Porphyromonas gingivalis at T1、T2、T3 after cryostorage.Results 6 and 12 weeks after treatment,PD,CAL,BOP value within the groups are lower than before treatment,group C PD,CAL,BOP indicators in 6 and 12 weeks after treatment are lower than the same period in group A and group B,the discrepancy has statistically significant(P<0.05);Pg content relative to total bacteria,In the treatment of 6 and 12 weeks,group C Pg content relative to the total bacteria is lower than that in group A and group B,the different between groups Pg content relative to the total bacteria have statistically significant(P<0.05);The curative effect of periodontal-endodontic combined lesions,after comprehensive treatment 12 weeks,group A compared with group B,group B compared with Group C,there was no statistically significant;group A compare with group C,the discrepancy has statistically significant(P<0.05).Conclusion Photo-Activated auxiliary treatment of periodontal-endodontic combined lesions curative effect is remarkable.
