摘 要:目的了解前牙开[牙合]畸形的颞下颌关节盘-突特征及髁突的位置关系,探讨其与开[牙合]的颅面特征的相关性。方法筛选门诊病例前牙开[牙合]青少年18例(女12例,男6例),年龄10~18岁,无正畸史,无全身性疾病,颞下颌关节可以无或者有主诉症状,或者有临床检查症状,拍摄治疗前患者头颅定位侧位片,双侧MRI,取36侧TMJ的磁共振(MRI)片,专人测量,分析盘-突及髁突位置关系,每患者的双侧MRI取平均值,同时在头影测量片选取代表颅面特征的项目,进行相关性分析。结果 Dp-Cc/Cc-Cs角与ODI有统计学差异P<0.05,有相关性,但其他各项指标之间均无相关性。结论①髁突在关节窝中的位置与前牙开[牙合]的颅面特征没有明显相关性。②但是前牙开[牙合]的垂直方向骨性因素越明显,提示需要更加关注颞下颌关节的盘-突关系是否正常。Objective To investigate the relationship between the TMJ and the craniofacial pattern occlusion in anterior open bite. Methods TMJ MRI, lateral cephalometric were taken respectively from 18 Chlidrens. The condyle position and disc position were measured. The correlation of condyle and disc position with craniofacial pattern and occlusion were analyzedmetrically and compared with relative test. Results There was statistical difference between Dp-Cc/Cc-Cs angle and ODI(P <0.05). Conclusion ① There was no significant correlation between the location of condyle and craniofacial features of anterior teeth open bite. ②However, the more obvious the osteogenic factors were in the vertical direction of the anterior teeth open bite,suggesting that more attention should be paid to the disc-condyle relationship of the TMJ.
