非接触广角观察系统辅助下外路视网膜脱离手术治疗裂孔源性视网膜脱离患者的临床疗效

摘　　要：目的观察非接触广角观察系统辅助下外路视网膜脱离手术治疗裂孔源性视网膜脱离患者的临床疗效。方法选取2015年1月至2016年12月我院收治的86例裂孔源性视网膜脱离患者(86只眼),根据治疗方法的不同将患者分为广角组(46例,46只眼)与显微组(40例,40只眼)。广角组患者行非接触广角观察系统辅助下外路视网膜脱离手术,显微组患者行显微外路视网膜脱离手术,均由同一手术者完成,对比两组患者的手术方式、视网膜裂孔封闭率和一次手术视网膜复位率、冷凝反应分级情况、手术前后视力情况、手术并发症发生情况。结果两组患者手术方式、视网膜裂孔封闭率和一次手术视网膜复位率、冷凝反应分级情况比较,差异均无统计学意义(P>0.05)。广角组患者术后视力情况较术前有所改善(P <0.05)。结论非接触广角观察系统辅助下外路视网膜脱离手术与显微外路视网膜脱离手术治疗裂孔源性视网膜脱离的手术效果相近,非接触广角观察系统辅助下外路视网膜脱离手术在改善患者视力方面略有优势。Objective To observe the clinical eff ect of external retinal detachment surgery assisted by non-contact wide-angle viewing system for patients with rhegmatogenous retinal detachment.Methods Eighty-six patients with rhegmatogenous retinal detachment(86 eyes)admitted to our hospital from January 2015 to December 2016 were selected,according to diff erent treatment methods,patients were divided into wide-angle group(46 cases,46 eyes)and microscopy group(40 cases,40 eyes).Patients in the wide-angle group underwent external retinal detachment surgery assisted by the non-contact wideangle viewing system,and patients in the microscopy group underwent micro-external retinal detachment surgery,all performed by the same operator.The surgical methods,retinal hole closure rate and retinal reattachment rate in one operation,grading of cold reaction,visual acuity before and after operation,and occurrence of surgical complications were compared between the two groups.Results There was no statistically signifi cant diff erence between the two groups of patients in terms of surgical methods,retinal hole closure rate,retinal reattachment rate,and grading of cold response(P>0.05).The postoperative visual acuity of the patients in the wide-angle group was improved compared with that before the operation(P<0.05).Conclusion The curative eff ect of external approach surgery by using noncontact wide-angle viewing system have the similar efficacy with the external approach microsurgery,got better visual acuity.External retinal detachment surgery assisted by non-contact wide-angle viewing system and micro-external retinal detachment surgery have similar surgical eff ects for the treatment of rhegmatogenous retinal detachment,the external retinal detachment surgery assisted by non-contact wide-angle viewing system has a slight advantage in improving patients'vision.
