摘 要:目的探讨磁共振成像(MRI)三维稳态构成干扰(3D CISS)序列在原发性三叉神经痛(PTN)三叉神经形态学改变中的应用价值。方法回顾性分析2017年4月至2019年2月亳州市人民医院PTN病人的影像及临床资料36例,运用3D CISS序列及多平面重组图像处理(MPR)技术,测量两侧三叉神经脑池段最大长度及面积、三叉神经-桥脑夹角及桥小脑角池截面,对其结果进行比较。结果三叉神经脑池段最大长度分别为(9.87±2.71)mm(症状侧)及(9.96±2.20)mm(非症状侧),二者比较差异无统计学意义(P>0.05);三叉神经脑池段最大截面分别为(27.28±8.59)mm2(症状侧)及(34.11±11.66)mm2(非症状侧),二者比较差异有统计学意义(P<0.05);三叉神经-脑桥夹角分别为(41.80±8.02)°(症状侧)及(47.75±11.01)°(非症状侧),二者比较差异有统计学意义(P<0.05);三叉神经桥小脑角池截面分别为(204.42±8.20)mm2(症状侧)及(229.19±58.47)mm2(非症状侧),二者比较差异有统计学意义(P<0.05)。结论3D CISS序列在评估PTN三叉神经脑池段形态学改变中,能给其定性及定量诊断;其中,三叉神经脑池段最大截面偏小、三叉神经-脑桥夹角锐利以及桥小脑角池截面狭窄时更易造成三叉神经痛的发生。Objective To investigate the application value of MRI three-dimensional steady-state interference(3 D CISS)sequence in the morphological changes of trigeminal nerve in primary trigeminal neuralgia(PTN).Methods Retrospective analysis was conducted on the imaging and clinical data of 36 patients with PTN in Bozhou people’s hospital from April 2017 to February 2019.The 3D CISS sequence and MPR technique were used to measure the maximum length and area of the trigeminal cistern segment,the trigeminal-pons angle and the pontocerebellar cistern cross-section.Then the results were compared.Results The maximum length of the trigeminal cistern segment was(9.87±2.71)mm(symptom side)and the maximum length was(9.96±2.20)mm(nonsymptomatic side).There was no significant difference between the two groups(P>0.05).The maximum cross-section of the trigeminal cistern segment was(27.28±8.59)mm^2(symptom side)and(34.11±11.66)mm^2(non-symptomatic side),and the difference was statistically significant(P<0.05).The angles of the trigeminal-pons were(41.80±8.02)°(symptom side)and(47.75±11.01)°(non-symptomatic side),and the difference was statistically significant(P<0.05).The cross-section of the pontocerebellar cistern of the trigeminal nerve was(204.42±8.20)mm^2(symptom side)and(229.19±58.47)mm^2(non-symptomatic side),and the difference was statistically significant(P<0.05).Conclusion The 3D CISS sequence could be used for qualitative diagnosis and quantitative diagnosis in the morphological changes of the PTN trigeminal cistern segment.Among them,trigeminal neuralgia was more likely to occur when the maximum cross section of the trigeminal cistern segment was small,the trigeminal-pons angle was sharp,and the cross section of the pontocerebellar cistern was narrow.
