妇科恶性肿瘤康复期患者情绪及生活质量团体心理治疗的随机对照试验

摘　　要：目的:探索团体心理治疗对妇科恶性肿瘤患者情绪及生活质量的影响。方法:采用随机对照研究设计。选取北京三级甲等肿瘤专科医院确诊为妇科恶性肿瘤(宫颈癌、子宫内膜癌或卵巢癌)患者60例,随机分为心理治疗组(n=30)接受为期6周、每周1次的团体心理治疗和对照组(n=30)接受团体患者教育。使用医院焦虑抑郁量表(HADS)、癌症患者生命质量测定量表(FACT-G)、症状评估简表中文版(MSAS-SF)于基线、干预后6周和干预后3月(随访)测评。结果:心理治疗组脱落1例。干预后6周,心理治疗组的焦虑和抑郁得分低于对照组(P<0.01),生理状况和功能状况得分高于对照组(P<0.01);随访时,心理治疗组的焦虑、抑郁、心理症状得分以及总困扰指数低于对照组(P<0.05),生理状况和功能状况得分高于对照组(P<0.01)。结论:团体心理治疗能够帮助妇科恶性肿瘤患者改善焦虑及抑郁情绪,提高其生活质量。Objective:To examine the effect of group psychotherapy on emotions and quality of life in patients with gynecological cancer.Methods:Sixty patients with gynecological cancer were selected and assigned randomly into the psychotherapy group(PG)(n=30) and control group(CG)(n=30).The PG patients received weekly group psychotherapy for 6 weeks,while the CG patients received group patient education.The Hospital Anxiety and Depression Scale(HADS),Functional Assessment of Cancer Therapy-G(FACT-G),and Chinese version of the Memorial Symptom Assessment Scale Short Form(MSAS-SF)were used for assessment at baseline,6 weeks after intervention and 3 months after intervention(follow-up).Results:One PG patient withdrew.After the 6-week intervention,the scores of anxiety and depression were lower in PG than in CG(P<0.01),the scores of physical state and functional state were higher in PG than in CG(P<0.01).In the follow-up,the scores of anxiety,depression,psychological symptom and the global distress index were lower in PG than in CG(P<0.05),the scores of physical state and functional state were higher in PG than in CG(P<0.01).Conclusion:It suggests that group psychotherapy could improve emotion state and quality of life in patients with gynecological cancer.
