前期软组织扩张对垂直骨增量手术效果的影响

摘　　要：目的探讨前期行软组织扩张术对采用异体支架材料行垂直骨增量术术后并发症发生情况以及成骨的影响。方法实验对象为7只成年比格犬,在实验侧缺损下颌骨行软组织扩张术,对照侧不进行处理,6周后于双侧下颌骨利用异体支架材料行垂直骨增量手术,2周后记录双侧术区并发症发生情况,10周后对比格犬下颌骨行CBCT分析。结果实验侧垂直骨增量术后未出现软组织开裂等并发症,对照侧6只比格犬术区出现软组织开裂,并发症发生率显著高于对照组(P<0.05)。CBCT测量结果显示,双侧植骨区域宽度无统计学差异,实验侧植骨区域高度以及灰度值显著高于对照组(P<0.05)。结论利用异体支架移植物材料行骨增量手术结合前期软组织扩张术,显著降低了骨增量手术后并发症的发生率,改善软组织愈合,提高植骨区域新骨生成,可达到良好的骨缺损重建效果。Objective To investigate the clinical effect of preconditioning soft tissue expansion on vertical bone regeneration in dogs.Methods In 7 beagle dogs,the soft tissue expansion of the resorbed edentulous alveolar ridge was performed on the test side,and the control side was not treated.After 6 weeks,the vertical bone augmentation with alloplastic scaffolds was performed on the bilateral mandible.After 2 weeks,complications in the bilateral side were recorded,and CBCT analysis was performed after 10 weeks.Results Flap dehiscence did not occur in test sites and occurred in control sites in 6 beagles(P<0.05).The results of CBCT measurement showed that there was no significant difference in the width of the bilateral grafting bone area,but the height and the gray value in test sites was significantly higher than those of control sites(P<0.05).Conclusion Vertical bone augmentation with preconditioning soft tissue expansion improves soft tissue healing and new bone formation and may facilitate reconstruction of alveolar bone defect.
