摘 要:目的:挖掘中药治疗非酒精性脂肪性肝炎(NASH)的处方规律,阐释核心用药可能的作用机制,探索NASH的关键机制并发掘潜在药物。方法:搜集中药治疗NASH的临床文献,对药物频次、类别、性味归经、关联规则及聚类进行分析,挖掘用药规律,筛选核心用药。构建NASH靶点相互作用网络,对重要靶点行功能富集分析,阐释核心用药的作用机制和疾病关键机制;用DrugBank、CTD和TCMSP数据库探寻NASH的潜在药物。结果:纳入127篇文献,药物145味,补益药占比最多;寒性、苦甘辛味与归肝、脾、胃经药物使用最多;核心用药为决明子、丹参、山楂和泽泻,可能的药效机制是调节脂质代谢。NASH的重要靶点22个,集中在免疫炎症、脂质代谢和胰岛素抵抗相关通路;为NASH匹配到白藜芦醇等8种治疗化合物及若干潜在中药。结论:本研究总结了中药治疗NASH的处方规律,并针对NASH的重要靶点发掘潜在药物,为临床选药及新药研发提供思路与参考。Objective: To discover the prescription regularities of traditional Chinese medicine in the treatment of nonalcoholic steatohepatitis(NASH), explain the possible mechanism of core drugs, and explore the primary mechanism of NASH and potential drugs. Methods: Clinical literatures on traditional Chinese medicine in the treatment of NASH were collected. The drug frequency, category, properties, flavors and channel tropism were analyzed, the association rules and cluster analysis were performed to explore the prescription regularities, then the core drugs were screened. The PPI network of NASH targets was constructed, then the enrichment analysis was performed to explain the mechanism of action of core drugs and the key mechanism of NASH. DrugBank, CTD, and TCMSP databases were used to find potential drugs for NASH. Results: 127 literatures, 145 drugs were included, and the largest proportion of drugs was tonifying drugs. Drugs with Han,Ku,Gan and Xin were the most commonly used,the most commonly channel tropisms were liver, spleen and stomach. The core drugs were Semen Cassiae, Salvia Miltiorrhiza, Hawthorn and Alisma. There were 22 important targets of NASH, which were related to the immune inflammation,the lipid metabolism and insulin resistance pathways. 8 potential therapeutic compounds and some potential Chinese herbs were found. Conclusions: This study summarized the prescription regularities of traditional Chinese medicine in treating NASH, and explored potential drugs, which provids ideas for clinical and new drug development.
