摘 要:目的评价生物羊膜移植术治疗角结膜热灼伤及化学性伤的效果。方法回顾性研究。以济源市人民医院2017年3月至2019年3月眼热灼伤及化学性伤者53例(56眼)为研究对象,分为研究组与对照组。研究组26例(28眼),进行生物羊膜移植术;对照组27例(28眼),进行传统羊膜移植术。术后随访1年。结果研究组的手术耗时为(17.6±6.7)分钟,术后眼表组织上皮化时间为(21.3±7.8)天,均低于对照组的(32.3±7.5)分钟及(32.6±10.4)天(t=7.735,4.599,P=0.000,0.000)。研究组术后视力(logMAR)为0.36±0.23,与对照组0.42±0.44比较,差异无统计学意义(t=3.406,P=1.000)。研究组出现手术并发症者3例(10.71%)为睑球粘连;对照组出现手术并发症者6例(21.43%),包括1例羊膜下积液,1例结膜面肉芽组织,4例睑球粘连,并发症发生率高于研究组(χ2=0.254,P=0.002)。结论生物羊膜移植术有助于角结膜热灼伤及化学性伤的恢复,且具有较高的安全性。Objective To evaluate the efficacy of biological amniotic membrane transplantation for the treatment of keratoconjunctival thermal burn and chemical injury.Methods This was a retrospective study.Total of 56 eyes of 53 cases of ocular heat burn and chemical injury from Mar.2017 to Mar.2019 in Jiyuan People’s Hospital were collected.All patients were divided into the study group and the control group.The study group,28 eyes of 26 cases,underwent biological amniotic membrane transplantation and the control group,28 eyes of 27 cases,received traditional amnion transplantation.The followed up time was 1 year.Results The time consuming of operation and epithelialization of ocular surface tissues in the study group were(17.6±6.7)minutes and(21.3±7.8)days,respectively,which were significantly lower than the(32.3±7.5)minutes and(32.6±10.4)days in the control group(t=7.735,4.599,P=0.000,0.000).The visual acuity(logMAR)after surgery was 0.36±0.23 in the study group,and 0.42±0.44 in the control group,the difference was statistically significant between the two groups(t=3.406,P=1.000).In the study group,the symblepharon occurred in 3 cases(10.71%).In the control group,there were 1 case of subamniotic fluid,1 case of conjunctival granulation tissue,and 4 cases of symblepharon,with a total incidence of 21.43%.The incidence of complications in the control group was higher than that in the observation group(χ2=0.254,P=0.002).Conclusion The biological amniotic membrane transplantation is safe and effective for the recovery of keratoconjunctiva heat burn and chemical injury.
