摘　　要：目的评估超声乳化人工晶状体植入术治疗穿透性角膜移植术(PKP)后白内障的效果。方法回顾性分析宿迁市第一人民医院和徐州市第一人民医院2017年10月至2019年6月行晶状体超声乳化人工晶状体植入术的PKP术后白内障共15例(15眼),术后随访6个月,观察BCVA,角膜内皮细胞密度、角膜大角形细胞比例、中尖角膜厚度及并发症。结果术后BCVA较术前提高(P<0.05),角膜内皮细胞密度较术前减少(P<0.05),六角形细胞比例较术前降低(P<0.05),中央角膜厚度与术前差异无统计学意义(P>0.05)。并发症有虹膜出血3例(20%),角膜移植片水肿2例(13.33%),后发障3例(20%)。结论超声乳化人工晶状体植入治疗PKP术后白内障安全有效,但术中需注意保护角膜内皮细胞。Objective To evaluate the efficacy of phacoemulsification and intraocular lens implantation for cataract after penetrating keratoplasty(PKP).Methods In this retrospective study,15 eyes of 15 patients with cataract after PKP who underwent phacoemulsification and intraocular lens implantation from Oct.2017 to Jun.2019 in Suqian First Pepple’s Hospital and Xuzhou First Pepple’s Hospital were analyzed.All patients were followed up for 6 months after surgery.BCVA,the corneal endothelial cell density(ECD),the percentage of hexagonal endothelial cells(HEX%),central corneal thickness(CCT)and complication were observed.Results After surgery,the vision(BCVA)were improved(P<0.05),ECD were reduced(P<0.05),and HEX%were reduced compared with preoperation(P<0.05).The difference was not statistically significant in CCT before and after surgery(P>0.05).Surgical complications after surgery included 3 cases(20%)of iris hemorrhage,2 cases(13.33%)of corneal graft edema and 3 cases(20%)of posterior capsule opacification.Conclusion The phacoemulsification with intraocular lens implantation is safe and effective for the treatment of cataract after PPK,but it is necessary to pay attention to protect corneal endothelial cell during surgery.
