摘　　要：目的探讨西洛他唑片联合阿司匹林与氯吡格雷对脑血管病血管内介入治疗患者临床疗效的影响。方法2017年8月至2019年8月在我院进行出血性脑血管疾病血管内治疗的66例患者,按治疗方案差异分为研究组和对照组各33例,对照组予以阿司匹林联合氯吡格雷治疗,研究组在此基础上加用西洛他唑片。比较两组治疗前后血流动力学指标及凝血功能、复发率、出血性并发症、缺血性并发症。结果治疗后两组全血黏度切变率、血浆黏度、相对血液黏度切变度、血细胞比容、纤维蛋白原及血小板聚集率均较治疗前降低,且研究组下降程度大于对照组(P<0.05);两组治疗后活化部分凝血活酶时间(Activated partial thromboplastin time,APTT)、凝血酶原时间(Prothrombin time,PT)均上升,且研究组上升更明显(P<0.05);研究组血小板抑制率高于对照组(P<0.05);两组复发率及出血性并发症发生率比较差异无统计学意义(P>0.05),研究组缺血性并发症发生率低于对照组(P<0.05)。两组均未出现明显不良发应。结论使用西洛他唑片联合阿司匹林与氯吡格雷可提高脑血管病血管内治疗患者治疗效果,降低血管内治疗后缺血性并发症,值得推广应用。Objective To investigate the effects of cilostazol tablets combined with aspirin and clopidogrel on clinical effects of intravascular interventional treatment in patients with cerebrovascular disease.Methods Sixty-six patients treated with intravascular interventional treatment of cerebrovascular disease in our hospital from August 2017 to August 2019 were selected.According to the difference in treatments,they were divided into study group and control group,33 in each group.The control group was treated with aspirin and clopidogrel.The study group was treated with cilostazol tablets based on the control group.The clinical efficacy,hemodynamic parameters and coagulation function before and after treatment were compared between the two groups.Results After treatment,the whole blood viscosity shear rate,plasma viscosity,relative blood viscosity shear rate,hematocrit,fibrinogen,and platelet aggregation rate of the two groups were lower than those before treatment,and the decreasing in the study group was greater than that of the control group(P<0.05).After treatment,activated partial thromboplastin time(APTT)and prothrombin time(PT)were increased in the two groups,and the increase in the study group was more significant(P<0.05).The platelet inhibition rate of the study group was higher than that of the control group(P<0.05).There was no significant difference in the recurrence rate and the incidence of bleeding complications between the two groups(P>0.05).The incidence of ischemic complications in the study group was lower than that in the control group(P<0.05).There was no obvious adverse reaction in the two groups.Conclusion Cilostazol tablets combined with aspirin and clopidogrel can improve the therapeutic effect of endovascular treatment,and reduce the ischemic complications after endovascular treatment.It is worthy of popularization and application in clinic.
