摘　　要：目的探讨快速康复外科(ERAS)理念对进展期胃癌根治术后患者胃肠功能恢复情况的影响。方法回顾性分析2018年5月—2019年5月河南省人民医院胃肠外科一病区接收的符合纳入标准的进展期胃癌患者86例,分为ERAS组和常规组,各43例,两组患者均以膜解剖理论为指导,成功实施3D腹腔镜胃癌根治术,围手术期分别采用快速康复理念(ERAS)和常规干预,比较两组患者术后首次排气时间、首次排便时间、胃管拔出时间、肠鸣音恢复时间、下床活动时间、术后住院时间。结果 ERAS组患者首次排气时间、首次排便时间、胃管拔出时间、肠鸣音恢复时间、下床活动时间、术后住院时间[(1.89±0.65)d、(2.56±0.45)d、(1.66±0.41)d、(14.47±2.72)h、(1.16±0.29)d、(6.94±1.56)d]均低于常规组[(2.76±0.79)d、(4.89±0.68)d、(2.74±0.56)d、(25.32±3.89)h、(3.68±0.68)d、(10.98±1.81)d],均具有统计学意义(P<0.05)。结论快速康复外科(ERAS)理念用于进展期胃癌根治术后效果良好,有助于胃肠道功能的恢复。Objective To explore the effect of rapid rehabilitation surgery(ERAS) on the recovery of gastrointestinal function in patients with advanced gastric cancer after radical operation.Methods From May 2018 to may 2019, totally 86 patients with advanced gastric cancer who met the inclusion criteria were analyzed retrospectively. They were divided into ERAS group and conventional group, 43 cases in each group. Under the guidance of membrane anatomy theory, the patients in both groups successfully performed 3 D laparoscopic radical gastrectomy, and rapid rehabilitation concept(ERAS) and conventional nursing were adopted in perioperative period the time of first exhaust, first defecation, extubation of gastric tube, recovery of bowel sounds, time of getting out of bed and hospitalization were compared.Results In the ERAS group, the time to first exhaust, the time to defecate, the time to pull out the gastric tube, the α recovery time of bowel sounds, the time to get out of bed,thepostoperative hospital stay[(1.89±0.65)d,(2.56±0.45)d,(1.66±0.41)d,(14.47±2.72)h,(1.16±0.29)d,(6.94±1.56)d] were lower than the conventional group[(2.76±0.79)d,(4.89±0.68)d,(2.74±0.56)d,(25.32±3.89)h,(3.68±0.68)d,(10.98±1.81)d],all have statistical significance(P<0.05).Conclusion the idea of rapid rehabilitation surgery(ERAS) has a good effect on the recovery of gastrointestinal function after radical operation of advanced gastric cancer.
