摘 要:目的:探讨抽提透刺法对脑梗死模型大鼠的脑保护机制及对CXC趋化因子配体12-CXC趋化因子受体4(CXCL12-CXCR4)信号通路的作用。方法:选取清洁级健康雄性SD大鼠56只,随机分为对照组14只及造模组42只。造模组采用线栓法建立脑梗死模型,对照组予以假手术。将造模成功的大鼠随机分为模型组、针刺组及药物注射组,各14只。模型组及对照组分别给予生理盐水10 ml/kg,腹腔注射,1次/d;针刺组采用抽提透刺法针刺头部穴位,幅度为0.1~0.2 cm,行针30 s,留针30 min,1次/d;药物注射组给予脑蛋白水解物注射液10 ml/kg,腹腔注射,1次/d。各组大鼠均给药14 d。检测比较各组神经功能评分、脑组织微血管密度(MVD),血清血管内皮生长因子(VEGF)、基质金属蛋白酶-9(MMP-9)水平以及大脑皮质CXCL12、CXCR4蛋白相对表达水平。结果:与模型组比较,针刺组、药物注射组大鼠神经功能评分较低,且针刺组小于药物注射组,差异均有统计学意义(P<0.05)。与模型组比较,针刺组、药物注射组大鼠脑组织MVD较高,血清VEGF、MMP-9水平较高,大脑皮质CXCL12、CXCR4蛋白相对表达水平较高,且针刺组大于药物注射组,差异有统计学意义(P<0.05)。结论:抽提透刺法能有效改善脑梗死大鼠神经功能,提高缺血区微血管密度,其脑保护作用可能与上调CXCL12-CXCR4信号通路相关蛋白表达有关。Objective:To analysis the r brain protection mechanism of extraction and penetration acupuncture and its effect on CXC chemokine ligand 12-CXC chemokine receptor 4(CXCL12-CXCR4)signaling pathway in cerebra infarction model.Methods:56 clean healthy male Sprague Dawley(SD)rats were selected,and were randomly divided into control group with 14 rats and model building group with 42 rats.The model building group was established cerebral infarction model by using suture embolization method,rats in the control group received sham operation.The successful model of the rats were randomly divided into model group,acupuncture group and drug injection group,each with 14 rats.The model group and control group were respectively given physiological saline 10 ml/kg,by intraperitoneal injection;acupuncture group was treated with extraction and penetration acupuncture at acupoints on the head,the range was 0.1~0.2 cm,needling for 30 s and retaining the needle for 30 minutes;drug injection group was given cerebroprotein hydrolysate injection 10 ml/kg,by intraperitoneal injection.All rats in each group were treated for 14 d.The neurological function scores,cerebral microvessel density(MVD),serum vascular endothelial growth factor(VEGF),matrix metalloproteinase-9(MMP-9)levels and cortical CXCL12,CXCR4 protein expression levels were detected and compared.Results:Compared with model group,the neurological function scores were lower in acupuncture group and drug injection group,and acupuncture group less than drug injection group,the differences were all statistically significant(P<0.05).Compared with the model control group,the cerebral MVD were higher,serum VEGF and MMP-9 levels were higher,cortical CXCL12,CXCR4 protein expression levels were higher in acupuncture group and drug injection group,and acupuncture group more than injection group,the differences were statistically significant(P<0.05).Conclusions:Extraction and penetration acupuncture can effectively improve neurological function in rats with cerebral infarction,increase mi
