摘  要:[目的]探讨促血小板生成素(TPO)和缺氧诱导因子-1α(HIF-1α)对脑瘫(CP)高危儿的预测诊断价值。[方法]选取120例CP高危儿作为研究对象,另选取同时期健康新生儿35例为对照组。对CP高危儿进行随访,直至受试者确诊或排除CP。根据随访结果,将CP高危儿分为CP组(n=36)和非CP组(n=73);CP组根据严重程度分为轻度亚组(n=14)、中度亚组(η=10)和重度亚组(n=12)。比较各组血清TPO、HIF-1a水平,并绘制ROC曲线分析血清TPO、HIF-la对CP的诊断价值。采用Spearman直线相关性分析CP高危儿血清TPO、HIF-la水平的相关性。[结果]CP组血清TPO水平高于对照组而低于非CP组;对照组、非CP组、CP组间比较以及各亚组间比较,差异均有统计学意义(P<0.05)。CP组血清HIF-1a水平高于对照组和非CP组,非CP组又高于对照组,而对照组、非CP组、CP组间比较以及不同病情程度各亚组间比较,差异均有统计学意义(P<0.05)。ROC曲线分析显示,血清TPO诊断CP的曲线下面积(AUC)为0.785,敏感性为77.80%,特异性为79.45%,最佳cut off值为596.75 pg/mL;血清HIF-la诊断CP的AUC为0.779,敏感性为80.06%,特异性为80.56%.最佳cut-off值为5.72 ng/mL。Spearman直线相关性分析显示,CP高危患儿血清TPO、HIF-1α间存在负相关性(r=-0.334,P<0.001)。[结论]CP高危儿血清TPO和HIF-1a水平较正常健康新生儿表达升高,血清TPO、HIF-la水平变化在CP的发病机制中发挥着重要作用。【Objective】To investigate the predictive diagnostic value of thrombopoietin(TPO)and hypox ia inducible factor-lα(HIF-lα)in high-risk infants with.cerebral palsy(CP).【Methods】A total of 120 cases of CP high-risk infants was selected as the research object,and 35 cases of healthy newborns in the same period were selected as the control group.The high-risk infants were followed up until CP was diagnosed or excluded.According to the follow-up results.CP high-risk infants were divided into CF Group(n=36)and non CF Group(n=73);CP Groups was divided into mild subgroup(n=14),moderate subgroup(n=10)and severe subgroup(n=12).The levels of serum TPO and HIP-lα in each group were compared and the ROC curve was drawn to analyze the diagnostic value of serum TPO and HIF-Iafor CP.Spearman linear correlation was used to analyze the correlation between serum TPO and HIF-Ialevels in CP high-risk infants.【Results】The serum TPO level in CP Group was higher than that in control group but lower than that in non CP Group;there were significant differences among control group,non CP Group.CP Group and each subgroup(P<0.05).The serum HIF-1 a level in CF Group was higher than that in control group and non CP Group,and that in non CP Group was higher than that in control group.There were significant differences among control group»non CP Group,CP Group and subgroups with different disease severity(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of serum TPO in the diagnosis of CP was 0.785,the sensitivity was 77.80%,the specificity was 79.45%,and the best cut-off value was 596.75 pg/ml;the AUC of serum HIF lain diagnosing CP was 0.779,the sensitivity was 80.06%,the specificity was 80.56%,and the best cut-off value was 5.72 ng/ml.Spearman linear correlation analysis showed that there was a negative correlation between serum TPO and HIF-lα(r=-0.334,P<0.001).[Conclusion]The levels of serum TPO and HIF-lα in high-risk infants with CP are higher than those in healthy newborns.The changes of serum TPO and HI
