小型钛板颌骨内固定术与颌间结扎牵引术治疗下颌骨骨折患者的对比研究

摘　　要：目的对比分析小型钛板颌骨内固定术与颌间结扎牵引术在下颌骨骨折患者中的应用效果。方法回顾性分析我院60例下颌骨骨折患者临床资料(2016年3月至2019年2月),采用小型钛板颌骨内固定术的为固定组(n=34),采用颌间结扎牵引术的为牵引组(n=26),统计比较两组临床疗效、咀嚼功能、牙齿咬合力及干预前后软垢指数(DI)、菌斑指数(PI)。结果固定组治疗总有效率94.12%高于牵引组73.08%(P<0.05);术后3个月固定组咀嚼功能、牙齿咬合力高于牵引组(P<0.05);术后3个月固定组PI、DI评分低于牵引组(P<0.05)。结论与颌间结扎牵引术相比,小型钛板颌骨内固定术治疗下颌骨骨折临床疗效显著,可促进术后咀嚼功能恢复,口腔环境影响较小。Objective To compare and analyze the application of small titanium plate jaw internal fixation and intermaxillary ligation and traction in patients with mandible fractures.Methods The clinical data of 60 patients with mandible fractures in our hospital were retrospectively analyzed (March 2016~February 2019),the small titanium plate jaw internal fixation was the fixed group (n=34) and the intermaxillary ligation and traction was the traction group (n=26),the clinical efficacy,chewing function,teeth bite force,and soft soil index (DI) and plaque index (PI) of the two groups were statistically compared.Results The total effective rate of treatment in the fixed group was 94.12% higher than that in the traction group 73.08% (P<0.05);The chewing function and occlusal force of the fixed group were higher than that of the traction group at three months after operation (P<0.05);The PI and DI scores of the fixed group were lower than that of the traction group at three months after operation (P<0.05).Conclusion Compared with intermaxillary ligation and traction,small titanium plate and jaw internal fixation is effective in treating mandible fractures,it can promote the recovery of chewing function after operation,and the oral environment has less impact.
