摘  要:目的探讨喀什地区维吾尔族妇女宫颈癌DNA倍体定量分析与高危型人乳头状瘤病毒(HPV)表达的相关性。方法前瞻性选择2017年1月到2019年12月在喀什地区第一人民医院进行DNA倍体定量分析及高危HPV检查的喀什地区维吾尔族妇女280例作为研究对象,以组织学诊断结果为金标准,分别计算细胞DNA倍体定量分析和高危型HPV检测的诊断价值。结果280例妇女中,组织病理学诊断为正常68例,炎症30例,宫颈上皮内瘤变(CIN)Ⅰ级44例,CINⅡ级36例,CINⅢ级30例,宫颈癌72例。DNA倍体定量分析显示异常细胞未见64例,少见44例,可见60例,大量112例。聚合酶链式反应(PCR)显示HPV16/18 DNA阳性86例,阳性率为30.7%。Pearson相关分析显示喀什地区维吾尔族妇女宫颈癌DNA倍体定量分析与高危型HPV表达呈正相关性(r=0.422,P=0.016)。多因素非条件Logistic回归分析显示细胞DNA倍体分析阳性、高危型HPV阳性作为导致宫颈癌发生的主要因素(P<0.05)。结论喀什地区维吾尔族妇女宫颈癌DNA倍体定量分析与高危型HPV表达存在相关性,2者联合检查可提高宫颈癌的筛查效果。Objective To investigate the correlation between DNA ploidy quantitative analysis of cervical cancer and the expression of high-risk human papillomavirus(HPV)in Uyghur women in Kashi.Methods From Janurary 2017 to December 2019,280 cases of Uyghur women in Kashgar region who given quantitative analysis of DNA ploidy and high-risk HPV in the First People’s Hospital of Kashgar were selected as the research object.The histological diagnosis results were used as the gold standard,and were assessment the diagnostic values.Results The histopathological diagnosis were normal in 68 cases,inflammation in 30 cases,CIN I grade in 44 cases,CIN II grade in 36 cases,CIN III grade in 30 cases,and cervical cancer in 72 cases.Quantitative analysis of DNA ploidy showed that there were 64 cases of abnormal cells,44 cases of rare,60 cases of visible cases,and 112 cases of large number.PCR showed 86 cases of HPV16/18 DNA positive,the positive rate were 30.7%.Pearson correlation analysis showed that there were positive correlation between quantitative analysis of cervical cancer DNA ploidy and high-risk HPV expression in Uyghur women in Kashi region(r=0.422,P=0.016).Multi-factor unconditional logistic regression analysis showed that the positive DNA ploidy analysis and the high-risk HPV were the main factors leaded to cervical cancer(P<0.05).Conclusion There are correlation between the quantitative analysis of cervical cancer DNA ploidy and the expression of high-risk HPV in Uyghur women in Kashgar region.The joint examination of the two can improve the screening effect of cervical cancer.
