姜莉云教授治疗慢性胃炎的经验浅析

摘　　要：目的:总结云南吴氏扶阳学术流派继承人姜莉云教授治疗慢性胃炎经验,以期裨益临床。方法:通过收集分析临床案例,结合中医理论知识,总结姜莉云教授临证治疗慢性胃炎经验。结果:姜莉云教授诊察病情,偏重舌诊,认为舌诊最能客观反映病性及人体正气的盛衰、病邪的深浅。认为舌形的老嫩反映体内津液的亏盈,提示阳气蒸腾气化功能的强弱;舌苔的厚薄反映脾胃运化水湿功能是否正常,而脾胃运化水湿关键依赖于脾阳的充足与否;姜莉云教授尤其重视舌下脉络的诊察,认为舌下脉络反映的是气血运行畅通程度。对慢性胃炎的病机,认为关键在于脾肾阳虚,水寒土湿木郁不达;治疗上以补火生土,温肝和胃为法,兼以密阳,多以吴萸四逆汤加味治之。其主导思想为云南吴氏“扶阳学派”理论:“温阳扶正,阳密乃固”“温水潜阳,引火归元”,临床效果显著。同时老师临证用药灵活,多有发挥,善于运用药对增强疗效。另外,姜莉云教授在治疗方面改革剂型,注重调摄,发明“姜辛固本脐贴”外治法,选取神阙、关元、太冲、三阴交、足三里等穴位,进行穴位贴敷,以温肾培元,巩固疗效。结论:姜莉云教授运用扶阳理论治疗慢性胃炎的经验值得总结应用。Objective:To summarize the experience of Professor Jiang Liyun,heir of Fuyang academic school in Yunnan Province,in the treatment of chronic gastritis in order to benefit the clinic.Methods:Through years of learning from teachers,collecting and analyzing clinical cases and combining with the theoretical knowledge of traditional Chinese medicine,the experience of mentor in clinical treatment of chronic gastritis was summarized.Results:The tutor examines the disease and emphasizes the tongue diagnosis.It is considered that the tongue diagnosis could objectively reflect the disease and the rise and fall of human health,and the depth of disease evil.It is considered that the old and tender tongue reflects the loss of body fluid,suggesting the strength of Yang Qi(阳气)transpiration and gasification function,the thickness of tongue coating reflects whether the function of spleen and stomach transportation and dampness is normal,and the key of spleen and stomach transportation depends on the adequacy of spleen yang.The teacher especially attaches importance to the diagnosis of sublingual choroid,and thinks that the sublingual vein reflects the unobstructed operation of Qi and blood.For the pathogenesis of chronic gastritis,it is considered that the key lies in deficiency of spleen and kidney Yang,water cold soil and wet wood depression,and the treatment is replenishing fire and producing soil,warming liver and stomach,and achieving the harmony between Yin and Yang(阴阳),and most of them are treated with Wuyu Sini decoction(吴萸四逆汤).Its leading thought is the theory of"Fuyang School"(扶阳学派)in Yunnan:"warming yang and strengthening healthy Qi,the harmony between yin and yang is solid","warming water and latent yang,igniting fire to return",the clinical effect is remarkable.At the same time,the teacher is flexible in clinical medicine,and is good at the use of drugs to enhance the curative effect.In addition,the tutor reforms the dosage form in the treatment,pays attention to the adjustment,invents the e
