散光型人工晶状体植入术治疗白内障合并角膜散光患者的效果

摘　　要：目的:观察散光型人工晶状体植入术治疗白内障合并角膜散光患者的效果。方法:选取80例白内障合并角膜散光患者作为研究对象,按照随机数字表法分为观察组和对照组各40例。两组均接受超声乳化白内障摘除术治疗,在此基础上,观察组实施散光型人工晶状体植入术治疗,对照组实施非球面人工晶状体植入术治疗。比较两组治疗前后视力水平、散光度、术后视觉质量[调制传递函数截止频率(MTF cut off)、100%对比度下视力、20%对比度下视力、9%对比度下视力]水平和术后并发症发生率。结果:治疗后,观察组裸眼视力水平明显高于对照组,散光度低于对照组,差异有统计学意义(P<0.05);观察组术后3个月MTF cut off、100%对比度下视力水平、20%对比度下视力水平和9%对比度下视力水平均明显高于对照组,差异有统计学意义(P<0.05);两组术后均未发生并发症。结论:散光型人工晶状体植入术治疗白内障合并角膜散光患者,可提高视力水平和视觉质量水平,降低散光度,其效果优于非球面人工晶状体植入术治疗效果。Objective:To observe effects of astigmatic intraocular lens implantation in treatment of patients with cataract and corneal astigmatism.Methods:80 patients with cataract and corneal astigmatism were selected as the research objects,and were divided into observation group and control group according to the random number table method,each with 40 cases.Both groups received phacoemulsification and cataract extraction.On this basis,the observation group was treated with astigmatic intraocular lens implantation,while the control group was treated with aspheric intraocular lens implantation.The visual acuity level,astigmatism level,postoperative visual quality[MTF cut off,visual acuity under 100%contrast,20%contrast and 9%contrast]level and postoperative complication rate were compared between the two groups before and after the treatment.Results:After the treatment,the naked eye visual acuity of the observation group was significantly higher than that of the control group;the total astigmatism level was lower than that of the control group;and the differences were statistically significant(P<0.05).The MTF cut off and visual acuity under 100%contrast,20%contrast and 9%contrast of observation group were significantly higher than those of the control group 3 months after the surgery,and the differences were statistically significant(P<0.05).No complications occurred in the two groups.Conclusions:Astigmatism intraocular lens implantation in the treatment of the patients with cataract and corneal astigmatism can improve the visual acuity and visual quality,and reduce the level of astigmatism.Moreover,it is superior to aspheric intraocular lens implantation.
