脐尿管癌7例报告并文献复习

摘　　要：本研究主要探讨脐尿管癌的诊治及预后情况,并分享单中心经验。回顾性分析自2013年5月~2019年6月就诊于我院的7例脐尿管癌患者的临床资料,并结合文献进行讨论。7例患者中男6例,女1例,平均年龄59.1岁。其中5例因出现无痛性肉眼出血就诊,1例因体检发现盆腔占位就诊,1例因自觉腹部包块就诊。予5例患者行扩大性膀胱部分切除术(其中1例患者术后4个月复发,评估后行扩大性膀胱部分切除术+盆腔淋巴结清扫术),2例患者行扩大性膀胱部分切除术+盆腔淋巴结清扫术;共3例患者接受术后辅助化疗。随访时间7~80个月,6例患者无瘤生存,1例患者带瘤生存。脐尿管癌临床表现无明显特异性,治疗以手术切除为主。扩大性膀胱部分切除术大多能提供长期无瘤生存,是否常规行淋巴结清扫尚存争议,联合化疗的综合治疗可能对提高脐尿管癌的疗效以及改善其预后有价值。This article focuses on analyzing the clinical diagnosis,treatment and prognosis of urachal carcinoma,and sharing the experience of single center.From May 2013 to June 2019,7 patients with urachal carcinoma were recruited in our hospital in this retrospective study.Among the 7 patients,there were 6 males and 1 female,with an average age of 59.1(29-74)years.After pathological diagnosis,5 patients underwent extended partial cystectomy,2 patients underwent extended partial cystectomy plus pelvic lymph node dissection,and 3 of them received postoperative adjuvant chemotherapy.The follow-up period ranged from 7 to 80 months.Six of whom survived free of cancer,while 1 case recurred.The clinical manifestation of urachal carcinoma has no obvious specificity.Surgery is the main treatment,and extended partial cystectomy can provide long-term tumor-free survival.However,whether routine lymph node dissection should be done is still controversial.Surgery combined with chemotherapy may be valuable to improve the efficacy and prognosis of urachal carcinoma.
