高分辨率CT与双时相18F-FDG PET/CT在孤立性肺结节中的应用

摘　　要：目的分析高分辨率CT结合18F-FDG PET/CT双时相显像对孤立性肺结节的诊断效果。方法回顾性分析本院2019年1月—2020年1月所收治125例孤立性肺结节患者的临床资料,所有患者均接受了高分辨率CT与18F-FDG PET/CT双时相显像检查,将手术病理检查结果为诊断依据,观察18F-FDG PET/CT双时相显像对良恶性病变的鉴别作用,并比较单项检查与联合检查的临床诊断效能。结果恶性结节患者的早期SUV、延迟SUV与滞留指数均明显高于良性结节,差异有统计学意义(P<0.05)。同时,高分辨率CT联合18F-FDG PET/CT双时相显像对良恶性结节诊断的灵敏度、特异度与符合率均明显高于单独18F-FDG PET/CT双时相显像。结论高分辨率CT结合18F-FDG PET/CT双时相显像对孤立性结节的良恶性有较高鉴别诊断作用,不仅能够保证早期检出率,也能对临床治疗发挥指导作用。Objective To analyze the diagnostic effect of high-resolution CT combined with 18F-FDG PET/CT dual phase imaging on solitary pulmonary nodules.Methods The clinical data of 125 patients with solitary pulmonary nodules admitted to our hospital from January 2019 to January 2020 were analyzed retrospectively.All patients were received highresolution CT and 18F-FDG PET/CT dual phase imaging.The results of surgical pathology were used as the basis for diagnosis and 18F-FDG was observed PET/CT dual phase imaging in the differentiation of benign and malignant lesions,and to compare the clinical diagnostic efficacy of single examination and combined examination.Results The early SUV,delayed SUV and retention index of malignant nodules were significantly higher than those of benign nodules(P<0.05).At the same time,the sensitivity,specificity and coincidence rate of high-resolution CT combined with 18F-FDG PET/CT dual phase imaging in the diagnosis of benign and malignant nodules were significantly higher than those of 18F-FDG PET/CT dual phase imaging alone.Conclusion High resolution CT combined with 18F-FDG PET/CT dual phase imaging has a high differential diagnosis effect on the benign and malignant solitary nodules,which can not only ensure the early detection rate,but also play a guiding role in clinical treatment.
