摘　　要：目的:研究医用几丁糖对大鼠颞下颌关节滑膜炎的治疗效果。方法:选取30只健康雄性大鼠,在它们的颞下颌关节腔中注射100μL浓度为3%卡拉胶溶液,建立颞下颌关节滑膜炎大鼠模型;成模后的大鼠按照完全随机分配的方法分为空白组、对照组和实验组三组,每组各10只。其中对照组注射100μL透明质酸,实验组注射100μL几丁糖。一周后抽取三组大鼠关节液各50μL,用双抗体夹心酶联免疫法(ABC-ELISA)检测大鼠关节液中的炎症因子IL-1β、IL-6、IL-8及TNF-α水平,用荧光定量PCR(RT-qPCR)法检测关节软骨中Ⅱ型胶原蛋白和蛋白聚糖水平变化;注射后第4周将大鼠处死,取其注射侧颞下颌关节为标本,观察比较三组罹患颞下颌关节滑膜炎的大鼠在注射透明质酸和几丁糖后的体重变化和组织学表现的改变,并进行统计学分析,评价其疗效。结果:3组大鼠的颞下颌关节均明显出现了关节滑膜炎样的组织学病变,对照组和实验组关节软骨的组织学病变程度较空白组轻微并在治疗后有所明显修复;此外对照组及实验组关节液中IL-1β、IL-6、IL-8及TNF-α水平均明显低于空白组,实验组治疗效果显著;实验组Ⅱ型胶原蛋白和蛋白聚糖的mRNA与对照组对比表达明显增高;此外对照组和实验组大鼠体重较空白组均有所升高,而实验组比对照组体重增加更加显著。结论:颞下颌关节腔注射几丁糖或能成为一种新的治疗方案用于临床中治疗颞下颌关节滑膜炎。Objective:To study the therapeutic effect of medical chitosan on rat temporomandibular joint synovitis.Methods:The modeled rats were divided into blank group,control group and experimental group according to the completely randomized allocation method,with 10 rats in each group.The model of temporomandibular joint synovitis was established by injecting carrageenan into the temporomandibular joint cavity,followed by injection without drug.Double-antibody sandwich ELISA(ABC-ELISA)was used to evaluate the expression levels of inflammatory factors including IL-1β,IL-6,IL-8 and TNF-ααin the synovial fluid samples.RT-qPCR was used to detect the gene expression of type II collagen and proteoglycan in articular cartilage.At the fourth week after injection,the rats were sacrificed,and the injection side of the temporomandibular joint was taken as a specimen to observe and compare the weight and histology changes of the three groups of rats with temporomandibular joint synovitis after injection of hyaluronic acid and chitosan.Statistical analysis was used to evaluate their efficacy.Results:Articular synovitis-like histological lesions were found in the temporomandibular joints of all rats.The lesions of articular cartilage in the control group and the experimental group were milder than those in the blank group.And the lesions showed certain repair in the control group and experimental group compared to the blank group.The weight of the rats in the control group and the experimental group was lower than that in the blank group.The rat weight was significantly increased in control group compared to the blank group.While the rat weight was much higher in the experimental group compared to the control group.The protein level of IL-1β,IL-6,IL-8 and TNF-αin synovial fluid was higher in the blank group compared to the control group.The treatment further decreased the level of IL-1β,IL-6,IL-8 and TNF-ααin experimental group compared to the control group.Compared with the control group,the mRNA of typeⅡcollagen and prote
