摘 要:目的:探讨口轮匝肌解剖学重建术治疗先天性唇裂的疗效及对患者面部软组织发育的影响。方法:选择2015年5月至2018年7月在我院接受治疗的先天性唇裂患儿130例纳入本次研究,根据随机数字表法分对照组、观察组,各65例。对照组采用单纯唇裂修补术治疗,观察组患者采用口轮匝肌解剖学重建术治疗,对比两组临床疗效指标、面部软组织指标及上唇长度、鼻唇高度。结果:两组术中出血量、手术及住院时间无统计学差异(P>0.05);相比于对照组,观察组术后面型角(FCA)、下唇长(LLL)、下唇凸度(LLP)均显著降低,且上面部高(UFH)、上唇长(ULL)、上唇凸度(ULP)均显著升高(P<0.05);比较两组健侧上唇长度、鼻唇高度差异均无统计学意义(P>0.05);观察组术后患侧上唇长度、鼻唇高度均显著高于对照组,且差异均有统计学意义(P<0.05)。结论:口轮匝肌解剖学重建术可改善先天性唇裂患儿面部软组织发育状况,有助于患侧上唇长度及鼻唇高度恢复,术中出血量及住院时间均未明显增加,该术式安全可靠,值得应用推广。Objective:To investigate the effect of anatomical reconstruction of orbicularis oris muscle on congenital cleft lip and palate and its effect on facial soft tissue development.Methods:130 patients with congenital cleft lip admitted to our hospital from May 2015 to July 2018 were selected.All patients were divided into control group and observation group according to the random number table method,65 cases in each group.Patients in the control group were treated with simple cleft lip repair while patients in the observation group were treated with anatomical reconstruction of orbital round muscle.The clinical efficacy index,facial soft tissue index,upper lip length and nasolabial height were compared.Results:There was no significant difference in blood loss,operation time and hospitalization time between the two groups(P>0.05).The postoperative facial angle(FCA),lower lip length(LLL),and lower lip convexity(LLC)of the observation group after operation were lonwer than those of the control group,while the upper facial height(UFH),upper lip length(ULL),the upper lip convexity(ULP)were higher than that of the control group(P<0.05);There was no difference in the length of the upper lip and the height of the nose and lip between the two groups(P>0.05).The length of the upper lip and the height of the nose and lip of the observation group were higher than those of the control group(P<0.05).Conclusion:The anatomical reconstruction of the orbicularis oris muscle can improve the development of facial soft tissue in patients with congenital cleft lip,and contribute to the recovery of the length of the upper lip and the height of the nasolabial.The anatomical reconstruction of the orbicularis oris muscle will not significantly increase the intraoperative blood loss and hospitalization time,which is safe and reliable,and is worthy of application and promotion.
