难治性胃食管反流病患者中的胃蛋白酶原及胃泌素17水平与食管反流情况的研究

摘　　要：目的探讨难治性胃食管反流病(refractory gastroesophageal reflux disease,rGERD)患者中胃蛋白酶原(pepsinogen,PG)和胃泌素17(gastrin-17,G-17)与食管反流的关系。方法选取2015年4月至2019年3月就诊于我院消化内科的rGERD患者73例,行24 h pH-阻抗监测及血清PGⅠ、PGⅡ、G-17检测,比较反流正常及异常两组患者血清PGⅠ、PGⅡ、PGⅠ/Ⅱ、G-17水平,判断两者的关系。结果直立位时,混合反流正常组与异常组血清PGⅡ水平、弱酸反流正常组与异常组PGⅠ/Ⅱ比值、混合反流正常组与异常组PGⅠ/Ⅱ比值比较,差异均有统计学意义(P<0.05);仰卧位时,液体反流正常组与异常组血清PGⅠ水平比较,差异有统计学意义(P<0.05)。结论rGERD患者混合反流异常时,PGⅡ水平降低;液体反流异常时,患者PGⅠ水平增加。PGⅠ/Ⅱ比值在弱酸、混合反流异常时均呈下降趋势,而G-17的水平与rGERD反流情况无关。Objective To investigate the relationship between the esophageal reflux and serum pepsinogen(PG),gastrin-17(G-17)levels of refractory gastroesophageal reflux disease(rGERD)patients.Methods 73 rGERD patients were enrolled from Apr.2015 to Mar.2019.Esophageal 24-hour pH impedance monitoring were performed to evaluate the reflux situation,and evaluated the serum PG,G-17 levels of patients.The serum PG,G-17 levels of normal group and abnormal group were compared to study the relationship between them.Results In upright position,PGⅡlevels of normal group and abnormal group of weakly acid reflux were significantly different(P=0.016),PGⅠ/Ⅱratios of normal group and abnormal group of weakly acid reflux and mixed reflux were significantly different(P<0.05).In supine position,PGⅠlevels of normal group and abnormal group of liquid reflux were significantly different(P=0.044).Conclusion When mixed reflux of rGERD patients is abnormal,their PGⅡlevels decreases;when liquid reflux is abnormal,PGⅠlevels increases.PGⅠ/Ⅱratio decreases when weak acid reflux and mixed reflux are abnormal.G-17 level is not related with the abnormities of reflux.
