摘　　要：目的分析膳食结构对2型糖尿病患者并发高同型半胱氨酸血症(HHcy)的影响。方法选取112例2型糖尿病患者,根据有无HHcy分为HHcy组52例和同型半胱氨酸(Hcy)正常组60例,比较两组患者一般资料和血生化指标。调查患者膳食情况,采用Logistic回归模型分析2型糖尿病患者膳食情况与并发HHcy的关系。结果HHcy组血清总Hcy水平、收缩压、尿微量白蛋白/肌酐值高于Hcy正常组,估测肾小球滤过率低于Hcy正常组(P<0.05)。每日蔬菜摄入量<400 g、每周肉类摄入量<560 g或>1050 g是2型糖尿病患者并发HHcy的影响因素(P<0.05)。结论低蔬菜摄入量、不适宜的肉类摄入量是2型糖尿病患者并发HHcy的危险因素,收缩压和尿微量白蛋白/肌酐比值升高、估测肾小球滤过率降低可能也与2型糖尿病患者并发HHcy有关。Objective To analyze the effects of dietary patterns on type 2 diabetes mellitus(T2DM)patients complicated with hyperhomocysteinemia(HHcy).Methods A total of 112 T2DM patients enrolled were divided into HHcy group(n=52)and normal homocysteine(Hcy)group(n=60)according to the presence of HHcy,and patients′general data and blood biochemical indicators were compared between the two groups.A survey on diet condition was perform in the patients,Logistic regression model was used to analyze the relationship between diet condition and concomitant HHcy in patients with T2DM.Results The HHcy group exhibited higher serum total Hcy level,SBP and microalbminurine to creatinine ratio,and lower estimated glomerular filtration rate as compared with the normal Hcy group(P<0.05).Daily vegetable intake<400 g,weekly meat intake<560 g or>1050 g were the influencing factors of developing concomitant HHcy in patients with T2DM(P<0.05).Conclusion For patients with T2DM lower vegetable intake and inappropriate meat intake are the risk factors for developing concomitant HHcy,and increase in SBP and microalbminurine to creatinine ratio as well as reduction in estimated glomerular filtration rate may be related to incident HHcy.
