摘  要:目的通过X线容积成像技术探讨在线红外定位系统在鼻咽癌放射治疗中的应用价值。方法建立回归模型分析放疗红外定位系统对反光定位球位置变化反应的准确度,运用X线容积成像技术与放疗红外定位系统同时对2016年8月至2017年12月我院初治的19例鼻咽癌肿瘤患者摆位误差进行测量,比较患者用OPS摆位与普通摆位所用时间和误差。结果反光定位球位置变化值与OPS测得值成线性关系,线性关系常数b1在三维方向分别为0.999、0.998、1。运用XVI测量在X、Y、Z轴向摆位平移误差分别为(1.54±0.97)、(1.33±0.99)、(1.55±0.94)mm,摆位旋转误差分别为(1.61±0.94)°、(1.49±0.93)°、(1.62±0.84)°;运用OPS测量在X、Y、Z轴向摆位平移误差分别为(1.61±1.01)、(1.43±0.93)、(1.66±0.96)mm,摆位旋转误差分别为(1.52±0.88)°、(1.48±0.87)°、(1.41±0.82)°,两种摆位误差测量方法在摆位平移误差与旋转误差两方面比较均无统计学差异(P>0.05)。摆位所需时间统计可见普通摆位所用时间为(134.63±26.38)s,OPS摆位所用时间为(139.31±28.35)s,P>0.05。结论在线放疗红外定位系统的运用既可以避免患者低剂量照射,提高摆位效率,还可以及时对患者位置进行调整,有利于提高治疗精确度。Objective To determine the value of X-ray volume imaging(XVI)based online infrared positioning(OPS)system in radiotherapy of nasopharyngeal carcinoma.Methods A regression model was established to analyze the response accuracy of the infrared radiotherapy positioning system to the location change of the reflective positioning ball.The X-ray volume imaging techniques and infrared radiotherapy positioning system were then used in 19 patients with nasopharyngeal carcinoma(NPC)who underwent initial treatment in our hospital between August 2016 and December 2017,for measurement of positioning errors.The positioning time and errors in NPC patients with OPS vs conventional positioning were compared.Results The location change in the reflective positioning ball was linearly correlated to the OPS-measured values,with the linear constants b1 being 0.999,0.998 and 1,respectively,on three dimensions.Using XVI measurement,the translational errors in the X,Y,and Z axes were(1.54±0.97)mm,(1.33±0.99)mm,and(1.55±0.94)mm;the rotational errors were(1.61±0.94)°,(1.49±0.93)°,and(1.62±0.84)°.Using OPS measurement,the translation errors in the X,Y,and Z axes were(1.61±1.01)mm,(1.43±0.93)mm,and(1.66±0.96))mm,the rotation errors of the position are(1.52±0.88)°,(1.48±0.87)°,and(1.41±0.82)°.There was no statistical difference between the two methods for measurement of translation and rotation errors of the positioning(P>0.05).The elapsed time was(134.63±26.38)s for conventional positioning vs(139.31±28.35)s for OPS positioning(P>0.05).Conclusion Using the online infrared positioning system for radiotherapy may avoid low-dose irradiation in patients,improve the positioning efficiency,and allow for readily adjustment of the patient position in favor of the treatment accuracy.
