颈动脉超声联合血清MMP-12和IL-33对动脉粥样硬化脑梗死的诊断价值

摘　　要：目的探究颈动脉超声联合血清MMP-12和IL-33对动脉粥样硬化脑梗死的诊断价值。方法选取2017年6月至2019年7月收治的缺血性脑卒中患者61例作为观察组,同时选取同期健康者50名作为对照组。所有纳入研究对象行颈动脉超声,测定颈动脉内膜中膜厚度;酶联免疫吸附试验测定血清IL-33和MMP-12的浓度。采用受试者诊断曲线评估各指标的诊断性能。结果ASCI患者血清IL-33浓度显著下降,而MMP-12则异常上调,且IMT增厚、斑块发生和中重度狭窄发生的比例均高于健康对照组(P<0.05)。诊断性能评价结果发现颈动脉超声诊断敏感性在单个指标中最高为81.9%,IL-33则具有最高的阳性预测值为79.9%,MMP-12特异度最高为75.8%。联合诊断其诊断性能指标如灵敏度、特异度、阴性、阳性预测值以及曲线下面积均显著升高。结论颈动脉超声联合血清IL-33和MMP-12可显著提高ASCI的诊出率。Objective To explore the diagnostic value of carotid ultrasound combined with serum MMP-12 and IL-33 in atherosclerotic cerebral infarction.Methods 61 patients with ischemic stroke from June 2017 to July 2019 were selected as the observation group,and 50 healthy people in the same period were selected as the control group.The carotid intima-media thickness was measured by carotid ultrasonography,and the serum levels of IL-33 and MMP-12 were measured by enzyme-linked immunosorbent assay.Subject diagnostic curve was used to evaluate the diagnostic performance of each index.Results Serum IL-33 concentration was significantly decreased in ASCI patients,while MMP-12 was abnormally increased,the proportions of IMT thickening,plaque and moderate to severe stenosis were higher than thoes of healthy control group(P<0.05).The evaluation of diagnostic performance showed that the highest sensitivity of carotid ultrasound was 81.9%,the highest positive predictive value of IL-33 was 79.9%,and the highest specificity of MMP-12 was 75.8%.The diagnostic performance indexes such as sensitivity,specificity,negative and positive predictive value and area under curve of combined diagnosis were significantly increased.Conclusion Carotid ultrasound combined with serum IL-33 and MMP-12 can significantly improve the diagnostic rate of ASCI.
