铸造基托结合排牙法在牙列缺损颌位记录中的临床应用

摘　　要：目的:铸造基托结合排牙法应用于牙列缺损颌位记录中,并评价其临床效果。方法:将24例临床病例随机分成实验组和对照组,每组各12例。实验组采用铸造基托结合排牙法制取颌位记录,对照组采用蜡堤[牙合]记录法制取颌位记录,并记录正中关系位准确率、颌位记录时间、初戴调合时间及咬合自如调合次数。结果:在正中关系位准确率上,实验组100%,对照组83.33%(P>0.05);在颌位记录时间上,实验组5.5(4.25-6.75)min,对照组18.5(17.0-20.75)min(P<0.001);在初戴调合时间上,实验组5.0(4.0-6.0)min,对照组10(8.25-13.0)min(P<0.001);在咬合自如调合次数上,实验组1至4次分别为1(8.33%)、5(41.67%)、5(41.67%)、1(8.33%),对照组分别为0(0%)、2(16.67%)、7(58.33%)、3(25.00%)(P>0.05)。结论:铸造基托结合排牙法制取颌位记录准确、简便、减少临床椅旁时间,在临床上值得尝试。Objective:Mental casting base with arrangement of teeth was applied to jaw relation record of dentition defect and its clinical effect was evaluated.Methods:24 clinical cases were randomly divided into experimental group and control group,with 12 cases in each group.In the experimental group,the jaw position was recorded by mental casting base with arrangement of teeth,and in the control group,the jaw position was recorded by wax embankment occlusal recording.The accuracy-rating of centric relation position,the time of jaw relation recording,the time of the first wearing occlusal adjustment,the numbers of occlusal freely bite were recorded.Results:On the accuracy-rating of centric relation position in the experimental group and the control one respectively was 100%,83.33%(P>0.05).The average time of jaw relation record in the experimental group was 5.5(4.25-6.75)min,while that of the control was 18.5(17.0-20.75)min(P<0.001).In terms of the frequency of the occlusal adjustment(from 1 to 4),the experimental group respectively was 1(8.33%),5(41.67%),5(41.67%),1(8.33%),while that of the control was 0(0%),2(16.67%),7(58.33%),3(25.00%)(P>0.05).Conclusion:Mental casting base with arrangement of teeth in the jaw position record is accurate,simple,reducing clinical time,which is worth trying.
