不同手术方法治疗翼状胬肉的美容效果探讨

摘　　要：目的:探讨不同手术方法治疗翼状胬肉的美容效果。方法:回顾性分析2016年1月-2017年12月收治的104例翼状胬肉患者临床资料,根据手术方式将患者分为结膜组(带角膜缘上皮的自体结膜移植术,n=54)和羊膜组(羊膜移植术,n=50)。两组分别实施手术,比较两组手术时间、角膜创面修复时间,评估患者术前、术后6个月时泪膜功能,随访观察患者术后6个月美容效果,记录两组术后并发症发生率。结果:结膜组手术时间长于羊膜组,角膜创面修复时间短于羊膜组,差异有统计学意义(P<0.05)。术后6个月,两组BUT、SIT均高于术前,且结膜组高于羊膜组,差异有统计学意义(P<0.05)。两组治愈、显效及复发率比较差异无统计学意义(P>0.05)。两组眼部刺激症状、巩膜溶解软化、肉芽肿及总并发症发生率比较差异无统计学意义(P>0.05)。结论:不同术式治疗翼状胬肉美容效果、术后复发率相近,带角膜缘上皮的自体结膜移植术更有利于促进术后角膜创面恢复、改善泪膜功能。Objective To investigate the cosmetic effects of different methods on pterygium surgery.Methods The clinical data of 104 patients with pterygium admitted between January 2016 and December 2017 were retrospectively analyzed.The patients were divided into the conjunctival group(autologous conjunctival transplantation with limbal epithelium,n=54)and the amniotic membrane group(amniotic membrane transplantation,n=50)according to the surgical methods.The surgery was performed in the two groups respectively.The operative time and repair time of corneal wounds were compared between the two groups.The tear film function was evaluated before surgery and at 6 months after surgery.The cosmetic effects of patients at 6 months after surgery were followed up and observed.The incidence rates of postoperative complications were recorded in the two groups.Results The operative time in the conjunctival group was longer than that in the amniotic membrane group,while the repair time of corneal wounds in the conjunctival group was shorter than that in the amniotic membrane group,the differences were statistically signifi cant(P<0.05).At 6 months after surgery,the BUT and SIT in the two groups were higher than those before surgery,and the conjunctival group was higher than the amniotic membrane group(P<0.05).There was no significant difference in cure rate,marked effect rate and recurrence rate between the two groups(P>0.05).There was no signifi cant difference in the incidence of eye irritation,scleral lysis and softening,granuloma and total complications between the two groups(P>0.05).Conclusion The cosmetic effects and postoperative recurrence rate of different treatments for pterygium are similar.Autologous conjunctival transplantation with limbal epithelium is more effective in promoting postoperative corneal wound recovery and improving tear film function.
