开窗减压联合塞治器对颌骨囊肿患者远期疗效与复发率的影响

摘　　要：目的:探究开窗减压联合塞治器治疗颌骨囊肿的远期疗效及复发率。方法:将笔者医院2016年1月-2018年1月收治的94例颌骨囊肿患者作为研究对象,根据患者治疗方式将其分为两组:A组(n=63),采用开窗减压+塞治器治疗;B组(n=31),完整刮治术治疗,比较两组围术期相关指标、术后术区疼痛情况、治疗有效率及手术并发症,术后随访2年,统计两组复发率。结果:A组手术时间显著短于B组(P<0.05),术中出血量显著少于B组(P<0.05);A组术后术区疼痛人数显著少于B组,且术区VAS评分显著低于B组(P<0.05);A组治疗有效率显著高于B组(P<0.05);两组术后感染及面型坍塌发生率无显著性差异(P>0.05),但A组神经并发症及肿胀发生率显著低于B组(P<0.05);术后随访2年,A组复发1例,B组复发4例,两组复发率比较无显著性差异(χ2=1.591,P=0.207)。结论:开窗减压联合塞治器在治疗大型颌骨囊肿中具有较好的应用效果,能有效缩小囊肿面积,且术后并发症少,患者治疗痛苦轻。Objective To investigate the long-term effect and recurrence of fenestration decompression combined with cyst plug for jaw cyst.Methods 94 patients with jaw cyst in our hospital from January 2016 to January 2018 were enrolled,and divided into two groups according to different treatment methods;Group A(n=63)received fenestration decompression combined with cyst plug,while group B(n=31)received complete curettage.Then the perioperative indicators,postoperative pain,treatment effi ciency,and surgical complications were compared between groups,and the recurrence rate of two groups was recorded during the postoperative 2 years follow-up.Results The operation time of group A was significantly shorter than that of group B(P<0.05),and the intraoperative blood loss was significantly less than that of group B(P<0.05).The proportion of postoperative pain in operation area was significantly lower in group A than in group B,moreover,the VAS score in operation area was also significantly lower in group A than in group B(P<0.05).The effective rate of treatment in group A was significantly higher than that in group B(P<0.05).The incidence rate of postoperative infection and facial depression had no difference between groups(P>0.05),while the incidence of neurocomplications and swelling in group A was significantly lower than that in group B(P<0.05).Two years follow-up showed that there was one recurrence in group A and four in group B,with no signifi cant difference between groups(χ2=1.591,P=0.207).Conclusion Application of fenestration decompression combined with cyst plug can effectively reduce the area of the cyst for patients with jaw cyst,which has the advantages of fewer postoperative complication and lower pain degree.
