种植体规格对基于种植体支持的下颌KennedyⅠ类可摘局部义齿修复效果的对比研究

摘　　要：目的:观察不同种植体规格对基于种植体支持的下颌KennedyⅠ类可摘局部义齿的修复效果。方法:选取笔者医院2018年1月-2019年1月收治的92例下颌KennedyⅠ类牙列缺损患者,随机分为对照组和观察组,每组46例。对照组患者在两侧第一磨牙区植入常规大小种植体,观察组患者在两侧第一磨牙区植入微型种植体。比较两组种植体稳定系数(ISQ)、骨丢失情况、植入成功率、不良反应发生率及患者满意度。结果:观察组术后即刻、术后12个月ISQ均略低于对照组,但两组比较差异无统计学意义(P>0.05);观察组术后12个月骨丢失低于对照组,两组比较差异有统计学意义(P<0.05);术后12个月观察组牙种植体植入成功率97.83%,高于对照组的86.96%,两组比较差异有统计学意义(P<0.05);术后12个月观察组并发症发生率6.42%,低于对照组的21.74%,两组比较差异有统计学意义(P<0.05);观察组患者满意度95.65%,高于对照组的80.43%,两组比较差异有统计学意义(P<0.05)。结论:采用即刻加载微型种植体对基于种植体支持的下颌KennedyⅠ类下颌可摘局部义齿修复效果良好,修复成功率高,不良反应发生率低,值得临床推广使用。Objective To compare the effect of immediate-load mini dental implants and conventional-size dental implants to retain mandibular Kennedy class I removable partial dentures.Methods 92 patients with mandibular Kennedy class I defect in our hospital from January 2018 to January 2019 were enrolled,and randomly divided into two groups,each with 46 cases.Conventional-size dental implants were placed in the first molar region on both sides of patients in control group.Mini dental implants were placed in the first molar region on both sides of patients in observation group.The implant stability quotient(ISQ),bone loss,implant success rate,adverse reactions and patient satisfaction were compared between groups.Results ISQ values of observation group were lower than those of control group immediately and 12 months after operation,with no statistic difference(P>0.05).The bone loss at postoperative 12mon in observation group was signifi cantly lower than that in control group(P<0.05).The success rate of dental implant placement in observation group was slightly lower than that in control group at 12 months after surgery,with statistic difference(97.83%vs 86.96%,P<0.05).The incidence rate of adverse reactions in observation group was slightly lower than that in control group at 12 months after surgery(6.42%vs 21.74%,P<0.05).The satisfaction rate of observation group was slightly higher than that of control group(95.65%vs 80.43%,P<0.05).Conclusion Application of immediate-load mini dental implants to retain mandibular Kennedy class I removable partial dentures has high success rate and low adverse reaction rate.
