医联体内护士PICC维护知识掌握现况及其影响因素分析

摘　　要：目的探讨医联体内护士外周置入中心静脉导管(peripherally inserted central catheter,PICC)维护知识掌握现况及其影响因素,为医院管理者探索医联体内"传帮带"的人才帮扶体系提供依据。方法 2019年3—6月,采用便利抽样法,选择本院医联体内32家医疗单位的120名护士为研究对象,对其进行现况调查,了解医联体内护士PICC维护知识掌握现况及其影响因素。结果共有111名护士完成调查,PICC维护知识总分(29.12±5.85)分,得分率为66.18%,合格及以上占53.15%(59/111);护士PICC维护知识各维度得分率从高至低依次为PICC日常维护、健康宣教、冲管及封管、并发症的识别及处理;医院级别、医院类型、工作年限是医联体内护士PICC维护知识掌握的独立影响因素(均P<0.05),共同解释变异的19.7%。结论医联体内护士PICC维护知识掌握现况欠佳,尤其是并发症的识别及处理掌握率最差;其中二级及以下与专科医院护士知识掌握较差,工作年限越长者其知识掌握越好。提示,医院管理者应积极探索如何将优质教学资源下沉至医联体内其他医疗单位,并充分发挥配备有PICC导管门诊医疗机构的优势,结合不同级别及类型的医院,制定个性化、合理化、多样化的培训方案。Objective To investigate the current situation and influencing factors of peripherally inserted central catheter(PICC)maintenance knowledge of nurses with the medical alliance to provide a basis for hospital managers to explore the talent supporting"mentoring"system for the medical alliance.Method From March to June 2019,120 nurses from 32 hospitals with the medical alliance were selected as research objects by convenient sampling method to investigate their current situation and understand the status and influencing factors of PICC maintenance knowledge of nurses with the medical alliance.Results A total of 111 nurses completed the survey.The total score of PICC maintenance knowledge was(29.12±5.85),the score rate was 66.18%and the qualified rate and above accounted for 53.15%.The dimensions of nurse's PICC maintenance knowledge scored from high to low included daily maintenance,health education,flushing and sealing,identification and treatment of complications.The level of work unit,type of work unit and years of service were independent influencing factors of current PICC maintenance knowledge of nurses with the medical alliance(P<0.05),which accounted for 19.7%of the variance.Conclusions Nurses in the medical alliance are poor for the mastery of PICC maintenance knowledge,among which the recognition and treatment rate of complications is the worst,among which the nurses in the second-level and below hospitals are poor in the mastery of knowledge.The longer they work,the high level their knowledge is at.Therefore,hospital administrators should actively explore how to deploy high-quality teaching resources to other medical units within the medical alliance,give full play to the advantages of outpatient medical institutions equipped with PICC catheters,and formulate individualized,rational and diversified training programs in combination with different levels and types of hospitals.
