阑尾低级别黏液性肿瘤的CT诊断及鉴别诊断

摘　　要：目的研究分析阑尾低级别黏液性肿瘤的CT表现特点及鉴别诊断,为临床提供诊断依据。方法选取2015年11月至2019年5月本院收治经病理证实为阑尾低级别黏液性肿瘤患者11例进行回顾性分析,所有患者术前均接受CT平扫及增强扫描,分析阑尾低级别黏液性肿瘤的CT影像学特点。结果10例低级别黏液性肿瘤CT表现为圆形、椭圆形改变,8例单房囊性,2例多房囊性占位,最大径5.9 cm,囊内CT值为14~28 Hu,囊内密度均匀,囊壁厚度欠均匀,5例囊壁伴条状或斑点状钙化。增强扫描囊壁及分隔轻度强化。1例合并急性阑尾炎,原发灶破裂形成腹膜假性黏液瘤,腹腔转移。结论螺旋CT检查在诊断阑尾低级黏液性肿瘤具有一定的特异性表现。Objective To study the CT features and differential diagnosis of low-grade mucinous tumor of the appendix(LAMN),and to provide the basis for clinical diagnosis.Methods Eleven patients with low-grade mucinous tumor of the appendix confirmed by pathology were reviewed in our hospital from November 2015 to may 2019.All patients underwent CT examination and enhanced scan.To summarize the CT features of low-grade mucinous tumors of the appendix.Results The CT imagings of 10 cases of low-grade mucinous tumors were round,oval like single room changes in 8 cases,and multiple compartment involvement in 2.the maximum diameter was about 5.9 cm,the CT value in the capsule was 14~28 Hu,the density in the capsule was still uniform,the thickness of the capsule wall was not uniform,Five cases showed spot/strip like shape of the lesion calcification.Mild enhanced was seen at the cyst wall and septa.The other case was complicated with acute appendicitis.The primary rupture resulted in pseudomyxoma of the peritoneum,with abdominal metastasis.Conclusion CT examination and enhanced scan have a certain specificity in the diagnosis of low-grade mucinous tumors of the appendix.
