摘  要:目的探讨首发精神分裂症血清白介素-6(IL-6)、钙结合蛋白(S100β)、神经营养因子3(NT-3)浓度与其认知功能评分、阳性与阴性症状量表(PANSS)评分的相关性。方法选取2015年4月—2017年1月该院收治的89例首发精神分裂症患者作为研究对象,根据其主要症状分为阳性组(n=48)和阴性组(n=41),同期80例体检健康者为对照组。采用酶联免疫吸附法测定白介素-6(IL-6)、钙结合蛋白(S100β)、神经营养因子3(NT-3)浓度;采用认知功能成套测验共识版(MCCB)进行认知功能评估;采用PANSS评分对不同症状、情绪等进行评估。结果阴性组和阳性组IL-6、S100β浓度明显高于对照组,NT-3明显低于对照组(P<0.05);同时阴性组IL-6浓度明显高于阳性组(P<0.05)。阴性组和阳性组连线测验(TMT)评分明显高于对照组(P<0.05),而符号编码(BACSSC)、言语记忆(HVLT-R)、空间广度(WMS-ⅢSS)、迷宫(NAB)、视觉记忆(BVMT-R)、语义流畅性(CF)评分明显低于对照组(均P<0.05)。IL-6浓度与PANSS一般症状评分呈正相关(r=0.347,P<0.05);NT-3浓度与PANSS总分和PANSS一般症状评分呈正相关(r=0.366,P<0.05)。阳性组IL-6浓度与BVMT-R评分呈正相关(r=0.377,P<0.05);NT-3浓度与BVMT-R评分呈正相关(r=0.412,P<0.05)。结论首发精神分裂症患者部分认知功能障碍与IL-6、S100β、NT-3浓度相关,以阳性症状为主和以阴性症状为主的患者不同的病理机制基础不同;神经细胞营养不足可能会导致以阳性症状为主的首发精神分裂症患者异常兴奋,以阴性症状为主的首发精神分裂症患者神经炎症反应强于以阳性症状为主的患者。Objective To investigate the correlation of the concentration of interleukin-6(IL-6),recombinant S100 calcium binding protein B(S100β),neurotrophin-3(NT-3),and scores of cognitive function,and Positive and Negative Syndrome Scale(PANSS)in first-episode schizophrenia.Methods Eighty-nine cases of first-episode schizophrenia and 80 cases of healthy individuals in Rizhao Mental Health Center from April 2015 to January 2017 were selected.The concentrations of IL-6,S100β and NT-3 were detected by enzyme-linked immunosorbent assay(ELISA).The systematic evaluation tool-MCCB was applied to assess cognitive function.Various symptoms and emotions were evaluated by PANSS.Results The control group had lower concentrations of IL-6 and S100β as well as higher NT-3 levels than those of other two groups(P<0.05).The IL-6 concentration in the negative group was significantly higher than that in the positive group(P<0.05).The Trail Marking Test(TMT) scores of negative group and positive group were significantly higher than those of the control group(P<0.05),while the scores of symbolic coding(BACSSC),speech memory(HVLT-R),spatial extent(WMS-IIISS),maze(NAB),visual memory(BVMT-R) and semantic fluency(CF) were significantly lower than those of the control group(P<0.05).The IL-6 concentration was positively correlated with the PANSS general symptom score(r=0.347,P<0.05).The NT-3 concentration was positively correlated with the PANSS total score and the PANSS general symptom score(r=0.366,P<0.05).The IL-6 concentration in positive group was positively correlated with BVMT-R score(r=0.377,P<0.05);NT-3 concentrationwas positively correlated with BVMT-R score(r=0.412,P<0.05).Conclusion Partial cognitive impairment in patients with first-episode schizophrenia is associated with the concentrations of IL-6,S100β and NT-3.Patients with positive symptoms and negative symptoms have different pathological mechanisms,moreover,nerve cell undernutrition may lead to abnormal excitability in first-episode schizophrenia patients with positive sympt
