原发性胃肠道外间质瘤12例临床病理特征分析

摘　　要：目的:分析原发性胃肠道外间质瘤(extra-gastrointestinal stromal tumor,E-GIST)的临床病理特征。方法:对南通大学附属医院病理科诊断的12例原发性胃肠外来源胃肠间质瘤患者的临床及随访资料进行回顾性分析。结果:胃肠外间质瘤具有与胃肠道间质瘤(GIST)相似的组织病理学特征,主要由梭形细胞组成,肿瘤细胞排列成束状或席纹状,细胞核呈棒杆状,间质可变性及出血。免疫组化结果显示,12例DOG1阳性,11例CD117阳性,11例CD34阳性。结论:胃肠外间质瘤是类似胃肠道间质瘤的罕见间叶组织肿瘤,病理组织学和免疫组织化学是诊断E-GIST的金标准和关键。以手术治疗为主,甲磺酸伊马替尼的作用尚不清楚,预后差,有必要进行更多研究,以充分了解其病理机制,制定更好的治疗方案。Objective:To analyze the clinicopathological features of 12 cases with primary extra-gastrointestinal stromal tumor(E-GIST).Methods:The clinical and follow-up data of 12 cases with primary extra-gastrointestinal stromal tumor were retrospectively analyzed,which were from the Department of Pathology,the Affiliated Hospital of Nantong University.Results:The extra-gastrointestinal stromal tumors had the same histopathological characteristics as GIST.The tumor was mainly composed of spindle cells.The tumor cells were arranged in fascicular,whorled,and storiform patterns.The nucleus was rod-shaped,and hemorrhage was visible.Immunohistochemical results showed that CD117,DOG1 and CD34 were positive in the tumor.Conclusion:The extra-gastrointestinal stromal tumor was a mesenchymal tumor similar to gastrointestinal stromal tumor.Histopathology and immunohistochemistry were the gold standard and the key for the diagnosis of E-GIST,which was mainly treated by surgery.The role of imatinib mesylate was not clear.As the tumor was rare,more research was needed to fully understand its behavior and promote better treatment.
