疏肝益肾方加减对乳腺癌患者免疫功能及生活质量的影响

摘　　要：目的观察疏肝益肾方加减对乳腺癌患者免疫功能及生活质量的影响。方法选取2013年6月—2016年7月青海省中医院收治的乳腺癌患者120例,随机分为对照组和观察组各60例,对照组患者给予环磷酰胺、多柔比星、多西他赛常规放化疗,观察组则联合应用疏肝益肾方加减治疗,21 d为1个周期,治疗4个周期。观察2组患者治疗后临床疗效、T细胞亚群细胞因子及生活质量评分变化,统计2组不良反应发生情况。结果观察组治疗总有效率为85.0%(51/60)明显高于对照组的70%(42/60),差异有统计学意义(P<0.05)。治疗后对照组CD3^+、CD4^+、CD4^+/CD8^+均明显下降(P均<0.05),CD8^+明显升高(P<0.05),观察组治疗后各检测指标均无明显变化(P均>0.05);治疗后2组SF-36生理及心理健康各项评分均明显升高(P均<0.05),且观察组均明显高于对照组(P<0.05);治疗过程中观察组食欲减退、腹泻、中性粒细胞下降、失眠、骨髓抑制、血小板下降发生率均明显低于对照组(P均<0.05)。结论疏肝益肾方加减应用于乳腺癌治疗中可提高疗效,显著改善患者免疫功能,提高患者生活质量,且有助于减少不良反应。Objective It is to observe the effects of Shugan Yishen Decoction on the immune function and quality of life in patients with breast cancer.Methods A total of 120 patients with breast cancer treated in Qinghai Provincial Hospital of Traditional Chinese Medicine from June 2013 to July 2016 were selected and randomly divided into a control group and an observation group with 60 cases in each group.The control group was treated with conventional radiotherapy and chemotherapy with cyclophosphamide,doxorubicin,and docetaxel,and the observation group was treated with Shugan Yishen Decoction based on the control group.21 days as 1 cycle and both groups were treated for 4 cycles.The clinical efficacy,and the changes of T cell subgroup cytokine and quality of life score of patients in the two groups were observed after treatment,and the occurrence of adverse reactions in the two groups was counted.Results The total effective rate of treatment in the observation group was 85.0%(51/60),which was significantly higher than that 70%(42/60)in the control group,the difference was significant(P<0.05).After treatment,the CD3^+,CD4^+,CD4^+/CD8^+in the control group were decreased significantly(P<0.05),and CD8^+was increased significantly(P<0.05),there was no significant change in these indexes in the observation group after treatment(P>0.05).After treatment,the scores of every items of physical and mental health in SF-36 were significantly increased in the two groups(P<0.05),and the scores in the observation group were significantly higher than those in the control group(P<0.05).During the treatment,the incidence of lost appetite,diarrhea,and decreased neutrophils,insomnia,bone marrow suppression,and platelet decline were significantly lower in the observation group than those in the control group(P<0.05).Conclusion The application of Shugan Yishen Decoction in the treatment of breast cancer can improve the efficacy,significantly improve the immune function and the quality of life of patients,and is helpful for reducing adverse rea
