胃癌术后不同化疗时机对老年患者近远期预后的影响

摘　　要：目的:比较胃癌术后不同化疗时机对老年患者近远期预后的影响。方法:收集我院自2012年1月至2015年1月收治的胃癌患者110例,依照患者手术至开始化疗时间不同分为观察组(≤6周,n=50)和对照组(>6周,n=60),所有患者均予以随访3年,对比分析两组患者近远期的临床疗效。结果:与对照组相比,观察组术后3个月和6个月的生存质量评分明显较高(P=0.043,0.028);术后3年进行比较,观察组患者的复发率显著低于对照组(20.00%vs 40.00%,P=0.024),死亡率也显著低于对照组(38.00%vs 60.00%,P=0.022),且观察组患者完全缓解、部分缓解及病情稳定者总疾病控制率较对照组明显提高(P=0.030)。结论:对胃癌患者术后进行早期化疗有助于减少术后癌症复发率和降低死亡率,并提高患者的生存质量。Objective:To compare the short-term and long-term prognosis of elderly patients with different timing of chemotherapy after gastric cancer surgery.Methods:110 patients with gastric cancer admitted to our hospital from January 2012 to January 2015 were divided into observation group(≤6 weeks,n=50)and control group(>6 weeks,n=60),according to the time from operation to the beginning of chemotherapy.All patients were followed up for 3 years,and the short-term and long-term clinical effects of the two groups were compared and analyzed.Results:Compared with the control group,the quality of life score of the observation group was significantly higher at 3 and 6 months after operation(P=0.043,0.028).Compared with the control group,the recurrence rate of the observation group was significantly lower than that of the control group(20.00%vs 40.00%,P=0.024),and the mortality rate was significantly lower than that of the control group(38.00%vs 60.00%,P=0.022),and the patients in the observation group were completely recovered.The disease control rate of patients with remission,partial remission and stable condition was significantly higher than that of the control group(P=0.030).Conclusion:Early chemotherapy for patients with gastric cancer after operation is helpful to reduce the recurrence rate and mortality rate of cancer after operation,and to improve the quality of life of patients.
