合并宫颈癌的多原发恶性肿瘤临床分析并文献复习(附7例)

摘　　要：目的:探讨合并宫颈癌的多原发恶性肿瘤(multiple primary malignant neoplasms,MPMN)的临床特点、诊治及预后。方法:对2014年8月至2016年12月于我院肿瘤科就诊的7例合并宫颈癌的MPMN患者的临床资料进行回顾性分析,并进行文献复习。结果:合并宫颈癌的MPMN患者占同期收治宫颈癌患者的1.08%。宫颈癌-肺癌、宫颈癌-结直肠癌常见,宫颈癌-非霍奇金淋巴瘤、宫颈癌-胆囊腺癌少见。2例患者行宫颈癌根治术,术后辅以放化综合治疗;5例患者行宫颈癌根治性放疗联合全身化疗。7例患者的其他原发癌均行根治性治疗。患者第一原发癌确诊后中位生存时间33个月,第二原发癌确诊后中位生存时间23个月。结论:合并宫颈癌的MPMN应早期诊断,行宫颈癌根治术联合放疗、化疗,对于其他原发恶性肿瘤也应根治性治疗,可取得较好预后。Objective:To investigate the clinical features,diagnosis and treatment,and prognosis of multiple primary malignant neoplasms(MPMN)combined with cervical cancer.Methods:The clinical data of 7 patients with MPMN combined with cervical cancer who were diagnosed in oncology department of the hospital from August 2014 to December 2016 were retrospectively analyzed.And literature review was performed.Results:MPMN patients combined with cervical cancer accounted for 1.08%of cervical cancer patients who were admitted in the same period.Cervical cancer-lung cancer and cervical cancer-colorectal cancer were common,while cervical cancer-non-Hodgkin's lymphoma and cervical cancer-gallbladder adenocarcinoma were rare.There were 2 patients undergoing radical resection of cervical cancer,adjuvant therapy by comprehensive chemoradiotherapy.There were 5 patients undergoing radical radiotherapy of cervical cancer combined with systemic chemotherapy.The 7 patients with other primary cancers underwent radical treatment.The median survival time after confirming the first primary cancer and the second primary cancer was 33 months and 23 months,respectively.Conclusion:For MPMN combined with cervical cancer,early diagnosis,radical resection of cervical cancer combined with radiotherapy and chemotherapy should be performed.The radical treatment should also be performed for the other primary malignant tumors,thus obtaining relatively better prognosis.
