摘 要:目的了解宁波市社区精神卫生资源现况,为社区严重精神障碍患者管理服务提供参考和依据。方法 2019年3—6月,对宁波市10个区县的精卫办、基层医疗卫生机构和康复机构开展调查。结果 10个区县精卫办达到相关设置要求;基层精防人员学历普遍偏低,村/社区精防医生文化程度为高中以下者占40%;年龄大多为30~39岁,村/社区精防医生50岁以上者占30.1%;村/社区精防医生非在编比例达58.0%;初级职称占多数,专职从事社区精防工作的人员仅163人(8.3%);精神康复机构149家;年人均工作经费为0.9元,年龄结构老化,非在编比例高,流动性大;康复机构接收精神残疾康复患者的比例偏低,医院与社区康复机构未达到有效衔接;部分经费未做到专款专用。结论建议基层医疗卫生机构精防人员定岗、定责、定编,确保工作专职性,减少人员流动,提高康复机构接受精神残疾人的比例,提升精神疾病防治康复工作人员的专业能力。Objective To understand the current situation of resources of community mental health in Ningbo, and to provide reference and basis for the management and service of community patients with severe mental disorders. Methods From March to June of 2019, a survey was carried out in mental health offices, primary medical and health institutions and rehabilitation institutions in 10 districts and counties in Ningbo. Results Mental health offices in 10 districts and counties achieved the relevant requirements;the educational background of grass-roots mental health staff was generally low, and 40% of mental health staff in villages and communities had education level below senior high school;most of the staff were between 30-39 years old, and 30.1% of mental health staff in villages and communities were over 50 years old;the proportion of temporarily employed mental health staff in villages and communities was 58.0%;most of them had junior titles, only 163 people(8.3%) were engaged in community mental health prevention full-time;there were 149 rehabilitation institutions;with the average working expenses were 0.9 yuan per person each year, age structure tended to aging, the proportion of temporarily employed was high, and with a high job mobility;the proportion of rehabilitation institutions receiving mental disability rehabilitation was low, and there was no effective connection between hospitals and community rehabilitation institutions;parts of funds were not used for special purposes. Conclusion It is suggested that the posts, responsibilities and staffing in primary medical and health institutions should be determined, so as to ensure professional work, reduce the flow of the staff, increase the proportion of mental disordered patients accepted by rehabilitation institutions, and improve the professional ability of mental disease prevention and rehabilitation staff.
