摘　　要：目的 分析人血清血管内皮生长因子(sVEGF)在乳腺疾病中的表达,并探讨其对乳腺癌诊疗的指导意义.方法 选取苏州大学附属第一医院甲状腺乳腺外科2019年1月—2019年5月收治的48例乳腺良性疾病患者为对照组,100例乳腺恶性疾病患者为实验组,应用血管内皮生长因子测定试剂盒分别检测两组人群血清中的VEGF的含量,并应用统计学软件分析两组间VEGF的水平差异以及实验组中VEGF与各临床病理特征间的关系.结果 实验组sVEGF阳性表达率为50.0%,而对照组sVEGF阳性表达率为25.0%,差异有统计学意义(P=0.004);sVEGF的阳性表达与乳腺癌患者的年龄(P=0.045),肿瘤分期(P=0.028),Ki67表达情况(P=0.019)密切相关,即年龄越大,肿瘤分期越晚,Ki67表达水平越高,sVEGF阳性率越高,而与乳腺癌患者的BMI,绝经与否,肿瘤位置,肿瘤大小,腋窝淋巴结转移情况,ER、PR、HER2表达情况及组织学分类无关(P>0.05).结论 检测sVEGF水平有助于鉴别乳腺疾病的良恶性,并且对乳腺癌患者临床分期的判断有诊断意义,sVEGF可作为临床乳腺疾病诊疗的参考指标.Objective To analyze the expression of serum vascular endothelial growth factor(sVEGF)in breast diseases,and to explore its significance in the diagnosis and treatment in breast cancer.Methods 48 patients with benign breast diseases and 100 patients with malignant breast diseases were selected as the control group and the experimental group respectively.sVEGF content in human serum of the two groups was detected with VEGF test kit.The difference of sVEGF level between the two groups and the correlation of VEGF and the clinicopathological features in the experimental group were analyzed with statistical software.Results The positive expression rate of sVEGF in experimental group was 50.0%,while the positive expression rate of sVEGF in the control group was 25.0%,the difference was statistically significant(P=0.004);the positive expression of sVEGF was closely related to the age of breast cancer patients(P=0.045),tumor stage(P=0.028),and the expression of Ki67(P=0.019).That is,when the patient was older,the tumor stage was later,and the expression level of Ki67 was higher,the positive rate of sVEGF would be highe.However,BMI,menopause or not,tumor location,tumor size,axillary lymph node metastasis,the expression of ER,PR and HER2,histological classification were not related to the expression of sVEGF(P>0.05).Conclusion Detecting the level of sVEGF is helpful to distinguish the benign and malignant breast diseases,and it has diagnostic significance for the clinical staging of breast cancer patients.So sVEGF can be used as a reference index for clinical diagnosis and treatment of breast diseases.
