摘  要:目的探讨头颈部鳞状细胞癌(head and neck squamous cell carcinoma,HNSCC)早期常见血清诊断标志物的临床诊断准确性。方法检索PubMed、Science Direct、Scopus和Web of Science数据库中血清标志物作为HNSCC诊断指标的相关文献,根据纳入与排除标准筛选文献,提取相应研究数据,使用Stata 14软件进行网状Meta分析。结果纳入分析的26篇文献包含总样本量3473例,HNSCC患者2141例,非HNSCC患者1332例。亚组Meta分析结果表明单用血清诊断标志物β2-微球蛋白(beta 2-microgolbulin)和血浆脂质结合唾液酸(plasma lipid-bound sialic acid,LASA)的平均OR值分别为1.57(P<0.01)和2.13(P<0.01);联合应用血清标志物SCCA+EGFR+CyclinD1和SCCA+LASA-P的平均OR值分别为1.92(P<0.05)和2.03(P<0.01)。结论联合应用SCCA+EGFR+CyclinD1、SCCA+LASA-P或单用β2-微球蛋白和LASA具有较高的早期HNSCC临床诊断率。OBJECTIVE To explore the clinical diagnostic accuracy of common serum diagnostic markers of head and neck squamous cell carcinoma(HNSCC).METHODS PubMed,Science Direct,Scopus and Web of Science were used to search the literatures about the use of serum markers as diagnostic criteria for HNSCC.According to the criteria of literature inclusion and exclusion criteria,the necessary data were extracted.Stata 14 software was used to conduct a network meta-analysis.RESULTS The 26 articles included in the analysis included a total sample size of 3473 cases,2141 cases of HNSCC patients,and 1332 cases of non-HNSCC patients.The results of subgroup meta-analysis showed that the average OR(Odds Ratio) value of serum diagnostic markers β2-microgolbulin(beta 2-microgolbulin) and plasma lipid-bound sialic acid(LSA) was 1.57(P<0.01) and 2.13(P<0.01);The average OR values of the combined serum markers SCCA+EGFR+CyclinD1 and SCCA+LASA-P were 1.92 (P<0.05) and 2.03(P<0.01).CONCLUSION Combined application of SCCA+EGFR+CyclinD1,SCCA+LASA-P or single β2-microgolbulin and plasma lipid-bound sialic acid have a high clinical diagnosis rate for early HNSCC.
