逆行黏膜下整块剜切术治疗非肌层浸润性膀胱癌的疗效分析

摘　　要：目的探究逆行黏膜下整块剜切术治疗非肌层浸润型膀胱癌的治疗效果。方法随机选取2017年5月-2019年5月来该院治疗的进行等离子棒状电极经尿道膀胱肿物整块切除术的患者49例(研究组),以及进行常规经尿道膀胱肿瘤电切术治疗的对照组患者49例,观察比较治疗效果。结果研究组与对照组患者手术时间、术后冲洗时间、尿管留置时间、住院时间分别为(43.84±2.18)min、(59.15±3.29)min,(1.62±0.31)d、(2.51±0.42)d;(4.09±0.21)d、(5.39±0.34)d;(6.15±0.47)d、(7.28±0.31)d,差异有统计学意义(t=7.542、8.123、7.014、7.170,P<0.05);研究组患者术后24 h血红蛋白浓度(0.85±0.07)g/dL,对照组(1.09±0.06)g/dL,差异有统计学意义(t=6.389,P<0.05);研究组患者并发症发生率、术后6个月复发率为2.04%、4.08%,对照组为14.28%、20.41%,差异有统计学意义(χ^2=4.900、4.653,P<0.05)。结论与常规经尿道膀胱肿瘤电切术相比,离子棒状电极经尿道膀胱肿物整块切除术具有手术时间短、并发症发生率低、术后6个月复发率低等优点,且尿管滞留时间短、术后住院天数短,安全可靠。Objective To explore the therapeutic effect of retrograde submucosal enucleation in the treatment of non-muscular invasive bladder cancer.Methods 49 patients who came to the hospital for treatment between May 2017 and May2019 who underwent transurethral bladder mass resection with plasma rod electrodes were randomly selected(as the study group),as well as control group of 49 patients for conventional transurethral resection of bladder tumors,observe and compare the treatment effect.Results The operation time,postoperative washing time,catheter indwelling time,and hospitalization time of the study group and control group were[(43.84±2.18)min,(59.15±3.29)min],[(1.62±0.31)d,(2.51±0.42)d,(4.09±0.21)d,(5.39±0.34)d,(6.15±0.47)d,(7.28±0.31)d,the difference was statistically significant(t=7.542,8.123,7.014,7.170,P<0.05);the hemoglobin concentration of the study group(0.85±0.07)g/dL at 24 h after surgery,and the control group(1.09±0.06)g/dL,the difference was statistically significant(t=6.389,P<0.05);the study group had complications incidence of symptoms and the recurrence rates within six months after surgery were 2.04%,4.08%,and the control group were 14.28%,20.41%(χ^2=4.900,4.653,P<0.05).Conclusion Compared with conventional transurethral resection of bladder tumors,ionic rod electrode transurethral bladder mass resection has the advantages of short operation time,low complication rate,and low recurrence rate within 6 months after surgery,with short residence time,short postoperative hospital stay,safety and reliability,and clinical promotion value.
