肺硬化性血管瘤多层螺旋CT影像表现特点探讨(附18例分析)

摘　　要：目的探讨肺硬化性血管瘤(Pulmonary Selerosing Heman-gioma,PSH)的多层螺旋CT(Multi-slice spiral CT,MSCT)影像学表现特点。方法回顾性分析2013年2月-2019年12月,经手术病理证实为PSH的18例患者MSCT影像学表现及临床、病理表现。结果18例患者中,单发17例,多发1例(2个病灶),共19个病灶;位于右肺上叶6例,右肺中叶2例,右肺下叶4例,左肺上叶6例,左肺下叶1例;病灶形态呈结节型(直径≤3 cm)8例,肿块型(直径>3 cm)9例,毛玻璃样表现2例;病灶密度均匀10例,见斑点样钙化5例;边缘清楚15例,可见晕征5例,浅分叶8例,粗长毛刺2例,空气新月征9例,病灶内见空气支气管征4例;增强扫描轻度强化5例,中度强化10例,明显强化3例;不均匀强化12例,贴边血管征11例。结论PSH在MSCT影像表现以良性肿瘤特征为主,并具有一些的特征性的征象,仔细观察、分析CT影像表现,有助于提高临床诊断能力。Objective To explore the imaging features of Pulmonary Selerosing Heman-gioma(PSH)with multi-slice spiral CT(MSCT).Methods A retrospective analysis of the MSCT imaging and clinical and pathological manifestations of 18 patients with PSH confirmed by surgery and pathology from February 2013 to December 2019 were retrospectively analyzed.Results Among the 18 patients,17 cases were single,1 case was multiple(2 lesions),19 lesions in total;6 cases were located in the right upper lobe,2 cases in the right middle lung,4 cases in the right lower lobe,and left upper lobe 6 cases,1 case of left lower lobe;8 cases of lesions were nodular(diameter≤3 cm),9 cases were mass(diameter>3 cm),2 cases were ground-glass appearance;10 cases of lesion density were uniform,with spotted calcification 5 cases;15 cases with clear margins,5 cases with halo signs,8 cases with shallow lobes,2 cases with thick and long burrs,9 cases with air crescent signs,4 cases with air bronchial signs in the lesions;5 cases with enhanced scans with mild enhancement,10 cases of moderate enhancement,3 cases of obvious enhancement;12 cases of uneven enhancement,and 11 cases of welted vascular sign.Conclusion PSH is mainly characterized by benign tumors in MSCT imaging,and has some characteristic signs.Careful observation and analysis of CT imaging findings will help improve the clinical diagnosis ability.
