缺血性中风急性期风痰阻络证病人VEGF、Ang-1、Ang-2水平与预后的关系探讨

摘　　要：目的探讨缺血性中风急性期风痰阻络证病人血管内皮生长因子(VEGF)、血管生成素-1(Ang-1)、血管生成素-2(Ang-2)水平与预后的关系。方法收集88例缺血性中风风痰阻络证病人急性期(发病72 h内)的四诊信息判断证候要素,评定入院时、发病4 d、7 d、14 d、28 d美国国立卫生研究院卒中量表(NIHSS)评分及发病90 d改良Rankin量表(mRS)评分,采用酶联免疫吸附法测定缺血性中风病病人发病72 h血清VEGF、Ang-1、Ang-2水平;采用Logistic回归模型分析缺血性中风病人血清VEGF、Ang-1、Ang-2水平与预后的关系。结果缺血性中风风痰阻络证病人发病14 d及发病28 d预后与发病初期血清VEGF有关(P<0.05),发病90 d预后与Ang-2有关(P<0.05)。结论缺血性中风风痰阻络证病人急性期血清VEGF、Ang-2高水平者较低水平者预后好,临床可参考VEGF、Ang-2判断病人预后。Objective To observe the relationship between the levels of vascular endothelial growth factor(VEGF),angiogenin-1(Ang-1),angiogenin-2(Ang-2)and prognosis in patients with wind-phlegm obstruction syndrome in acute stage of ischemic stroke.Methods Four diagnostic information of ischemic stroke patients with wind-phlegm obstruction syndrome(within 72 hours)was collected dynamically to judge the syndrome elements.The scores of National Institutes of Health Stroke Scale(NIHSS)at 1 d,4 d,7 d,14 d and 28 d,and Modified Rankin Scale(mRS)at 90 d were collected dynamically.The levels of VEGF,Ang-1 and Ang-2 were measured by Enzyme-linked immunosorbent assay within 72 hours after onset of ischemic stroke.The relationship between the levels of VEGF,Ang-1,Ang-2 and prognosis in patients with ischemic stroke was studied by Logistic regression model.Results The prognosis of patients with wind-phlegm obstruction syndrome of ischemic stroke at 14 d and 28 d was related to the level of VEGF within 72 hours(P<0.05),and the prognosis at 90 d was related to the level of Ang-2(P<0.05).Conclusion The patients with wind-phlegm obstruction syndrome in acute stage of ischemic stroke had better prognosis especially the levels of VEGF and Ang-2 were higher than those with low levels,clinical reference of VEGF and Ang-2 could judge the prognosis of patients.
